A mechanic was injured when EE and another employee attempted to move a steel plate covering a below ground water valve. While sliding the heavy plate, it hung up and then moved suddenly pinching EE's left middle finger between the floor and plate. EE was treated for the injury and received four stitches on the tip of the finger.

An electrician and co-worker were lifting an empty pallet into the back of a truck when the pallet struck the truck body. The sudden stop resulted in the employee suffering a strain on bicep. Employee received surgery on 3-30-2022 which resulted in one day of lost time and was reportable.

Employee stated was looking into the top of #10 mixing tank in LCH, when employee opened up the hatch the pressure caused lch to blow into employee's face. Employee was wearing goggles, but the LCH is watery and ran under employee's goggles.

The employee stated the pin was removed from the arm so the actuator could be rotated up and out of the way to access the arm bushing. The mechanic had a hand on top of arm lowering it into place to reinstall the pin to actuator when weight of arm went over the center and pinched left index finger between the arm and housing. Broken finger was discovered on 2-14-2022

Removing the product valve to change out the air sweeps located in the bottom hopper of a hydrate product bulk trailer, the employee was flushed with a small amount of hydrate. Hydrate got in both eyes causing irritation. Right cornea received abrasion. Employee was prescribed antibiotic eye drops.

The employee stated Employee was replacing a broken rear leaf spring on a pickup. The spring slipped while placing on the differential and they attempted to catch it. While the employee did not get on restricted duties until 7/27/2021, so that is when became reportable.

EMPLOYEE STATED THAT WAS LIFTING DOOR TO CLOSE UP PREHEATER AND FELT PULL ON SHOULDER OUT OF PLACE.

Employee helping replace door on R1/R2 elevator when door fell cutting right hand. 5 stitches received

Employee stated that EE had dropped the lid of the premixer on EE's pinky finger. This resulted in a broken finger and a cut on EE's right hand.

Employee was attempting to remove a guard with assistance from another miner when EE felt a pop and burning in EE's lower abdomen.

Injured employee was working to repair a conveyor belt when right thumb was struck against a metal pulley fracturing right thumb.

The injured employee was working on 3/19/20 working to replace a return idler. The return idler became unsecured and fell downward. As it fell the idler struck the injured employee's left wrist between the handrail and the piece of equipment. The injured employee was diagnosed with a fractured left wrist on 4/9/20 with an MRI.

Conveyor roller smashed right ring finger causing an avulsion

EMPLOYEE STATED EMPLOYEE HAD LIME IN EYE WHILE EMPLOYEE WAS CHANGING FILTERS IN THE DUST COLLECTOR. EMPLOYEE WAS TAKEN TO THE OPTOMETRIST, WHO DISCOVERED BURNS INSIDE THE EMPLOYEE'S BOTTOM EYELID. A PRESCRIPTION WAS GIVEN AT THAT TIME.

An employee stated, the employee was carrying steel elevator buckets from a pallet to 25 feet away, handing them over a concrete wall to another employee. The employee felt a strain in their shoulder. From treatment, it was found there was a tear by a doctor on 11/5/2019 and will be receiving surgery soon. Date is not determined.

EMPLOYEE STATED WHILE REPLACING AN OIL FILTER ON THE #2 COMPRESSOR AT THE MISSISSIPPI VERTICAL, THE EMPLOYEE FELT AND HEARD A POP IN THE UPPER RIGHT ARM. THE INCIDENT OCCURRED OCTOBER 3RD 2019. FROM DIAGNOSIS, ON 12-31-2019 THE DOCTOR HAS RECOMMENDED SURGERY. THE SURGERY DATE IS TBD TO FIX RIGHT SHOULDER.

Employee was on a ladder running pipe when they turned around on ladder to measure pipe behind them. The ladder started falling and EE grabbed pipe with left arm causing their weight to pull against their shoulder, dislocating the left shoulder.

Employee was bagging pebble in the lime shed area when the pebble bagger choked up causing material to get into their right eye.

Opened the kiln door to take pilot light out and dust blew out the door and into eyes

EE bent over to pick up tools and hit mouth on the top of handrail. This resulted in a laceration through the lip and chipped teeth. Vision was partially obstructed by wearing goggles.

EMPLOYEE STATED THAT WHILE SECURING A PIPE THE EMPLOYEE LOOKED UP AND SOMETHING FELL INTO EMPLOYEE'S EYE. EMPLOYEE WAS TREATED AT A LOCAL EMERGENCY ROOM AS WELL AS A LOCAL EYE DOCTOR AND PRESCRIPTION MEDICATION WAS USED IN EYE.

Employee stated while cleaning the firing floor at RK1, that they felt something fall into their right eye. They were treated at the local emergency room where they were prescribed antibiotics for the eye.

Employee was tightening a pipe when hand slipped causing left pinky to be smashed. This injury resulted with a broken pinky and stitches.

Employee was running a communication line from second floor to seventh floor. While performing this task, employee got lime in right eye. First Aid was administered and eye was flushed, then later in the shift employee said eye was still irritated. Taken to the doctor where the eye was examined and flushed again. Employee was prescribed anti-biotic as a precaution.

Employee stated was filling a bulk bag and as employee was pushing on the bag felt pain in front left shoulder. Employee was seen by a physician and placed on light duty.

Employee was opening a railcar lid when they smashed a finger between the lid and a loading spout.

Employee stated employee was cleaning under the TSK lime belt when employee felt and heard a pop in employee's right knee. The employee was sent to a doctor, then a specialist and is schedule for surgery.

EMPLOYEE WAS CLEANING UP MATERIAL AND CARRYING A 5 GALLON BUCKET STRAINING THE RIGHT ELBOW.

Employee stated was running lime samples and when EE switched material from the crusher to the pulverizer the lime was hot and heated up the metal tray. EE burned hand on the bottom of the tray which resulted in a 2nd degree burn.

Employee stated was operating a haul truck and came upon a rock in the road. Employee got out of the cab and moved the rock by hand. Employee felt a sharp pain in abdomen. Was later diagnosed with a hernia and had surgery to repair. Began missing time on 07/27/2017.

Employee sliding electrical control panel when it shifted and pinched right pinky finger between control panel and concrete steps requiring sutures. Employee was released to return to work.

Injured employee was helping to change a lime screen when some lime dust fell behind employee's safety glasses into employee's left eye. Employee had antibiotics applied to employee's eye.

Employee stated was removing bags from the east transfer point dust collector on the fourth floor of the lime shed when the employee removed goggles fogged up. Employee removed them for cleaning and lime fell into the employee's eye. The eye doctor used a prescription to treat eye.

Employee stated employee was opening a valve when employee felt a pop. Employee was seen by a physician. The initial x-ray report was negative. On January 10th the employee was contacted by the doctor stating the x-ray show a fractured rib.

Employee stated while ee was taking off goggles lime fell off the goggles into eyes. EE was treated by a local eye doctor and a prescription medication was used in eyes.

EE was replacing a rotary feeder valve. As EE was drifting the valve out the valve shifted and mashed right middle finger causing a cut that required stitches.

Employee stated while throwing a rail switch EE felt a "pop" in left knee. The employee was sent to a doctor, then a specialist and is scheduled for surgery.

Employee stated that EE was climbing out of RK2 Halo Duct while holding onto the manhole door. EE stepped into a hole and dropped the door onto EE's left thumb causing a compound fracture and severe laceration. Employee was taken to a surgeon where the thumb was repaired. Employee is an Engineering Project Manager and will continue working full duty.

Employee stated that while ee was walking up a stairs ee left knee became painful and swollen. EE was taken to a doctor where ee was given light duty.

Employee stated EE was pushing a hopper under a scrubber when EE felt a "pop" and developed a bulge in EE's mid-section. EE was sent to a physician where EE was diagnosed with a hernia. EE has been sent to a surgeon to schedule the repair for the hernia.

Employee stated as EE was stepping out of #6 kiln EE caught EE's armpit on the bracket of the burner pipe. EE received 14 sutures to EE's left armpit.

Employee stated EE was pulling bags out of a bag house when EE felt tightness & pain in EE's groin. Employee was sent to a physician where EE was diagnosed with a hernia. Employee is scheduled for surgery on 10/18/2016.

Employee stated while working on the RK2 preheater ram EE was removing a 2" inch steel pin when it dropped onto right little finger. EE further stated it both smashed and cut finger. EE was taken to the hospital for treatment and the ER physician closed the wound with 2 sutures.

Employee stated that while cleaning scrubbers at MRH2 employee received lime burns on both wrists. The doctor ranked the lime burns as 2nd degree and placed ee on light duty.

The employee was working and burnt upper inside arm area with hydrate dust, resulting in a second degree burn.

Employee apparently walked in the spray of a high pressure sprayer. EE received a laceration on right upper leg requiring surgery. Employee is currently off work.

Employee stated that EE was changing a motor on the east lime screen and received lime burns on both forearms. Employee went for a follow up treatment on 07/07/2016 and a "new" doctor in the occupational medicine department re-classified the burns as 2nd degree. (Note-medical notes indicate re-classification of burns).
