A mechanic was injured when EE and another employee attempted to move a steel plate covering a below ground water valve. While sliding the heavy plate, it hung up and then moved suddenly pinching EE's left middle finger between the floor and plate. EE was treated for the injury and received four stitches on the tip of the finger.

An electrician and co-worker were lifting an empty pallet into the back of a truck when the pallet struck the truck body. The sudden stop resulted in the employee suffering a strain on bicep. Employee received surgery on 3-30-2022 which resulted in one day of lost time and was reportable.

Employee stated was looking into the top of #10 mixing tank in LCH, when employee opened up the hatch the pressure caused lch to blow into employee's face. Employee was wearing goggles, but the LCH is watery and ran under employee's goggles.

The employee stated the pin was removed from the arm so the actuator could be rotated up and out of the way to access the arm bushing. The mechanic had a hand on top of arm lowering it into place to reinstall the pin to actuator when weight of arm went over the center and pinched left index finger between the arm and housing. Broken finger was discovered on 2-14-2022

Removing the product valve to change out the air sweeps located in the bottom hopper of a hydrate product bulk trailer, the employee was flushed with a small amount of hydrate. Hydrate got in both eyes causing irritation. Right cornea received abrasion. Employee was prescribed antibiotic eye drops.

The employee stated Employee was replacing a broken rear leaf spring on a pickup. The spring slipped while placing on the differential and they attempted to catch it. While the employee did not get on restricted duties until 7/27/2021, so that is when became reportable.

The injured employee was working on 3/19/20 working to replace a return idler. The return idler became unsecured and fell downward. As it fell the idler struck the injured employee's left wrist between the handrail and the piece of equipment. The injured employee was diagnosed with a fractured left wrist on 4/9/20 with an MRI.

EMPLOYEE STATED EMPLOYEE HAD LIME IN EYE WHILE EMPLOYEE WAS CHANGING FILTERS IN THE DUST COLLECTOR. EMPLOYEE WAS TAKEN TO THE OPTOMETRIST, WHO DISCOVERED BURNS INSIDE THE EMPLOYEE'S BOTTOM EYELID. A PRESCRIPTION WAS GIVEN AT THAT TIME.

An employee stated, the employee was carrying steel elevator buckets from a pallet to 25 feet away, handing them over a concrete wall to another employee. The employee felt a strain in their shoulder. From treatment, it was found there was a tear by a doctor on 11/5/2019 and will be receiving surgery soon. Date is not determined.

EMPLOYEE STATED WHILE REPLACING AN OIL FILTER ON THE #2 COMPRESSOR AT THE MISSISSIPPI VERTICAL, THE EMPLOYEE FELT AND HEARD A POP IN THE UPPER RIGHT ARM. THE INCIDENT OCCURRED OCTOBER 3RD 2019. FROM DIAGNOSIS, ON 12-31-2019 THE DOCTOR HAS RECOMMENDED SURGERY. THE SURGERY DATE IS TBD TO FIX RIGHT SHOULDER.

Employee was on a ladder running pipe when they turned around on ladder to measure pipe behind them. The ladder started falling and EE grabbed pipe with left arm causing their weight to pull against their shoulder, dislocating the left shoulder.

EE bent over to pick up tools and hit mouth on the top of handrail. This resulted in a laceration through the lip and chipped teeth. Vision was partially obstructed by wearing goggles.

Employee was running a communication line from second floor to seventh floor. While performing this task, employee got lime in right eye. First Aid was administered and eye was flushed, then later in the shift employee said eye was still irritated. Taken to the doctor where the eye was examined and flushed again. Employee was prescribed anti-biotic as a precaution.

Employee stated was running lime samples and when EE switched material from the crusher to the pulverizer the lime was hot and heated up the metal tray. EE burned hand on the bottom of the tray which resulted in a 2nd degree burn.

Employee stated was removing bags from the east transfer point dust collector on the fourth floor of the lime shed when the employee removed goggles fogged up. Employee removed them for cleaning and lime fell into the employee's eye. The eye doctor used a prescription to treat eye.

Employee stated that EE was climbing out of RK2 Halo Duct while holding onto the manhole door. EE stepped into a hole and dropped the door onto EE's left thumb causing a compound fracture and severe laceration. Employee was taken to a surgeon where the thumb was repaired. Employee is an Engineering Project Manager and will continue working full duty.

Employee stated while working on the RK2 preheater ram EE was removing a 2" inch steel pin when it dropped onto right little finger. EE further stated it both smashed and cut finger. EE was taken to the hospital for treatment and the ER physician closed the wound with 2 sutures.

Employee stated that while cleaning scrubbers at MRH2 employee received lime burns on both wrists. The doctor ranked the lime burns as 2nd degree and placed ee on light duty.

Employee stated that EE was changing a motor on the east lime screen and received lime burns on both forearms. Employee went for a follow up treatment on 07/07/2016 and a "new" doctor in the occupational medicine department re-classified the burns as 2nd degree. (Note-medical notes indicate re-classification of burns).
