Showing all 19Miner lost footing while placing drill steel in holder, causing him to drop the steel, which bounced off ground and struck his left foot. Impact caused bruising to three toes on the foot. Safety boots not worn.
Worker opening and closing rail car hopper gates and felt pain in his back. Moved to a different job next day. Sought medical attention on 7/24/06 and placed on limited duty.
Miner was changing screen deck on 6/28/06 and felt slight pain in back but continued to work. Pain worse on 7/1/06 and sought medical attention. He was treated for pinched nerve/strain in his back.
Operator and mechanic were lifting pieces of sheet pile using an excavator. Mechanic was attaching load to a large wire rope sling. Other end of sling fell off teeth of excavator striking him on the head. Mechanic was wearing a face shield but no hard hat and sling was attached to teeth of the excavator instead of attached by a hook or proper lifting connection.
Monday morning (7/25/05) Employee was dispatched to another site and showed up complaing that he must have got a hernia on saturday at work. He went to the doctor and the doctor confirmed he did have a hernia and needed surgery. On saturday employee asked for help from laborer in lifting some but not all items. Did not complain that day of any pain.
EE and two co-workers were attempting to remove a large asphalt chunk off of a belt when the load shifted causing the asphalt chunk to come down and smash his right index finger. He sought medical attention and received 3 stitches.

UNLOADING CONVEYOR ROLLER FRAMES RIGHT HAND GLOVE WAS CAUGHT TWISTING RIGHT INDEX FINGER CAUSING A SPRAIN TO RIGHT INDEX FINGER.
