Employee was opening the main gate to the Butler Mine and injured right hand (two fingers). When the employee went to pick up the gate and move it aside and place the gate post in the holder, that gate slipped out of employee's hand and two fingers were pinched/injured.

Employee was removing a bearing housing from shaker screen drive side. When the housing broke free the housing slipped in the sling and turned causing the employees left pinky finger to become smashed between the housing and the platform.
Employee was helping move an 8" HDPE Pipe and strained his back.
Employee was removing a water gate at the Plant/Bins. When he pulled the board there was "a bee sting like feeling", in his right shoulder. He reported this to P.K. (supervisior). At the time he thought that he could "just work through it." He finished Friday and worked on Sat. Reported the injury to the safety director on 09/17/09 @ 10:10 a.m.
Employee was removing a jack shaft when chain slipped and caught his finger.
