On 03/23/2013 at about 0200 am an approximately 60 lb boulder struck employee on left elbow after apparently coming off of a conveyor. Employee returned boulder to conveyor and felt fine. on 03/24/2013 employee reported to his foreman that his left arm and elbow had shooting pains and numbness from finger tips to shoulder. Emergency room Dr. diagnosis of deep contusion.
While sweeping off conveyor deck by hand, ran hand across metal burr on deck. Cut right index finger.
At approx. 2:00pm employee noticed a irratation in his right eye,while crusher was in operation.
He was bending over to pick up a cone while drilling.
LIFTING & PULLING SCREEN OFF BOBCAT BUCKET.
SHE WAS TRYING TO LOOSEN A BOLT AND SHE PULLED A MUSCLE IN HER BACK.
A ROCK HIT HIS TEETH.
