EE set down an engine lifting spreader bar after using it. When unhooking the spreader bar rigging from the crane, it tipped over landing on the EE's left foot.

EE was standing on the edge of a tank at approx. 56" off the ground and rinsing down a tank with a fire hose. EE attempted to get down from the ledge and grabbed a piece of piping on the side. The piping broke loose and caused EE to lose balance and land awkwardly on both feet. EE sought medical treatment after the shift was complete and was diagnosed with a fractured left leg.

EE was moving furnace pallet car rail on the forks of the forklift, attempted to manually rotate it while on the forks. The rail slipped pinching the left index finger between the rail & forklift fork a laceration resulting in stitches.

Employee was working north side of P filter on Phase 2 filter floor. Employee attempted to clean the proxy switch on the filter. The employee's right pointer finger was contacted by the bolt on the gear causing a laceration.

Employee felt a pull in stomach while lifting an empty CO2 gas cylinder over a 1-inch lip on welding cart. The employee reported that in June 2018, had a non-work related herniated belly button repaired and thought may have damaged the repair.

Employee was installing one of two bushings on shaft and dropped the shaft from waist height onto EE's right foot. The shaft bent the steel portion of the safety toe on EE's right boot. The diagnosis was contusion of right foot and closed non-displaced fracture of distal phalanx of right great toe.

Employee was changing clothes prior to the end of their shift in locker room. While removing their pants, the employee's foot got caught in the pant leg. The employee fell over and struck their head on the edge of a locker. Employee received stitches and staples.

On July 11, 2018, employee reported to supervisor that employee's back was sore from prior shift. EE stated that there was no particular occurrence or event that caused discomfort. EE could not identify a specific incident but stated has prior ongoing back issues and articulated a gradual progression of symptoms occurred as EE performed repetitive hosing work.

Employee helped pull a suction hose that was stuck in the mud out of the Phase 1 Hydro tank. Employee reported that EE felt pain in EE's right shoulder. On February 14, 2018, employee notified the employer that EE had started physical therapy for EE's shoulder.

Employee was working with partner to replace the latch plate on a shovel dipper. As the employee and partner were setting down the new plate for final placement, the partner dropped the plate onto the employee's left hand striking the tip of the employee's left ring finger. Employee received and utilized a finger splint.

Employee attempted to return a crowd belt tensioning wrench on its hanging hook. As employee attempted to secure the wrench, it missed the hook and the wrench fell on the employee's left index finger, resulting in a fracture.

While exiting the 62 shovel, employee realized that ee forgot to turn on the MG set. The employee turned and caught the door latch as it was closing. EE left index finger got pinched between the door jam resulting in a laceration and fracture to the tip of ee left index finger.

Employee was assisting a forklift operator who was transporting a pipe. The pipe shifted and rolled over the employee's left hand while the load was still on the ground. The employee received left hand second - forth digit contusions. The employee underwent proximal interphalangeal joint capsular release with pinning of the joint surgery on October 31, 2018.

Employee is a pro truck driver. On March 17, 2017, EE was lifting/pulling cable on the labor crew. After performing this repetitive work, EE noticed pain in EE's rt shoulder. EE did not have pain at rest. On April 4, 2017, employee went in for evaluation. EE was released without restrictions and referred to PT. EE recently informed the employer that EE began the PT sessions.
