Showing all 23Miner was assisting in the removal of a boulder from a jaw crusher. While working, another smaller rock near the work was dislodged and fell, smashing the miner's finger between this rock and the boulder they were working to remove. The impact caused a laceration on the right middle finger, requiring stitches.
Miner was replacing V-belts (drive belts) for a Rolls crusher drive system. While performing this operation the miner's finger became stuck in between the drive sheave and the belt while rotating the sheave to put the belt in place. This lacerated the miner's right index finger. Miner's laceration was stitched at a local ER.
Setting a conveyor belt in place. The worker was placing blocking under the conveyor support when the load shifted downward, smashing the worker's Left thumb between the block and the conveyor leg. This cut the tip of the thumb and required stitches to close the wound.
Worker was helping to load a welding skid on a trailer. When the skid was lifted the load started to swing. The worker reached out Right hand to stabilize the load. The little finger of the hand was pinched between the skid and a truck scale already loaded on the trailer. This smashed the finger between the two objects and broke the skin on the little finger. Required stitches.
Worker was changing a v belt when EE's RT little finger was pinched between the belt and the sheave. The worker did not immediately report the incident thinking there was no injury. A later visit to a DR. revealed the finger is fractured. Worker reported injury July 9th.
ee bent over to pick up a 50 pound bag of shot powder & felt lower back pain. The pain continued to gradually get worse over the next few days.
Rolled hand into V-Belt Pulley, Fractured Knuckle.
EE was removing rock by hand from Jaw when a rock approx 18" in size shifted from above where left hand was placed pressing palm of hand onto sharp rock cutting/stab type wound in left palm. Took to hospital and received 23 stitches. EE was not wearing gloves at time of accident.
Shoveling around support structure of primary screen cleaning off concrete slab of built up dirt & rocks. Using a shovel to push shovel material when felt a pop in groin area. Felt a burning section right away contacted supervisor. Went to hospital for ultra sound. Found to have unilateral inguinal hernia. Follow up with doctor on Jan 10th if surgery or treatment for options.
Rock was to hot on the burner went back to take the pan off the burner and some of the rock started popping or exploding causing fragments of rock and very small fragments of metal from the pan to become airborne, one of the fragments of rusted metal went into right eye.
EE was changing screen on load out wash screen. While doing so he was in middle deck torching "J" bolts from upper deck. A metal slag from "J" bolt fell onto employee's face by his eyebrow and worked under safety glasses. We flushed eye out & took him to medical clinic where they flushed eye out and gave medical prescription cream for eye.
Employee was helping 2 other employees change a return roller on a stacker. He was lifting the belt and twisting while placing the roller. He was lifting approximately 20 pounds at the time. It affected his lower back which he has had issues before.

Changing cutting edges on loader and the bolt slipped and cut finger.
He stepped into the truck washout to install the metal divider and in doing so he received a chemical burn.
WHILE STACKING 4 X 4 STEEL TUBING, CUT RIGHT INDEX FINGER ON SHARP EDGE WHILE WEARING GLOVES.
PUSHED THE CRACKED CERAMIC RESERVOIR TOILET TOP WITH THE BACK OF HIS LEFT HAND & HIS MIDDLE FINGER WAS CUT BY THE CERAMIC.
INSTALLING A MOTOR GUARD ON THE CONE CRUSHER, EE LOST HIS STEP AND HIS LEFT HAND GOT CAUGHT BETWEEN THE MOTOR AND THE MOTOR GUARD, SMASHING HIS MIDDLE FINGER.
LOWER BACK STRAIN CAUSED BY LIFTING HEAVY OBJECTS OVER TIME WITHOUT ASKING FOR HELP. CHANGING SCREENS ON WASH PLANT IN ODD POSITIONS AND ALWAYS BEING BENT OVER LIFTING & TWISTING.
HE WAS CHANGING ONE SCREEN ON THE 6 BY 18 AND HE LIFTED THE SCREEN BY HIS SELF, AND HE FELT TESTICLES PAIN.
EE MOVING SCREENS IN DEBRIS BOX SLIPPED OUT OF HAND AND CAUSED A LACERATION ON HIS NOSE.
WHILE REMOVING THE RETAINER PLATE ON THE REAR BRAKES OF A 980F LOADER, THE PLATE CAME LOOSE AND FELL. IT PINCHED THE EE'S FINGER OF HIS LEFT HAND WHICH RESULTED IN THE LACERATION.
