G5 industrial Services was preparing to install a discharge pipe under the log-washers. The pipe had a ninety-degree elbow on one end that was facing upward while in the air. When the pipe was placed on the ground it rolled to the side, EE misjudged where the ninety-degree elbow would lay down to rest. The pipe landed on the top of EE's foot causing injury.

PM Technician completed their Pre-Shift Inspection on their truck at the beginning of their shift. The hood did not close properly, so the operator rocked it with their hands in an attempt for it to close properly. When the hood closed, it smashed the operator's right middle finger, causing a laceration on the top and bottom of the operator's finger requiring stitches.

On 5/15/24, EE removed a car jack stand from work area. Put their right hand around the center column, while using their left hand to remove the safety pin holding the center column. When pin was removed, the column dropped, causing right thumb to be smashed by the Saddle. EE did not report injury until 5/28/24. Took EE to have thumb evaluated and found it fractured.

Employee was getting welding rods out of the front right side toolbox on the welding truck bed. The employee was on top, kneeling on the toolbox while unlatching the door. The door started to swing downward away from the employee, and employee attempted to catch the falling door with right hand. The door smashed the employee's right ring finger tip.

Employee was unfolding a forklift attachment to move barrels, as EE unfolded the attachment EE felt a pain in EE's left arm. Employee reported to supervisor and attempted to treat with OTC meds until EE requested to seek a doctor on 8/7/2023 at which time it became reportable.

While removing/ attempting to throw a 20 lb rock from the cone crusher, the miner struck right index finger on a lifting eye with a sharp edge. EE was treated at the hospital diagnosed with partial amputation, returned to work same day, operated truck for 8 days until EE saw a hand surgeon 5/12 and was placed off work for 3 days returning 5/16.

Miner was leaning over pressing stomach against a head pulley drum performing return roller idler inspection, when EE straightened back up miner reports EE heard a pop in EE's stomach. EE was examined at a clinic the next day diagnosed with a umbilical hernia.

After detaching an impact idler assembly, the miner attempted to push the assembly through the center of the conveyor frame for removal. While doing so the miner pinched and lacerated right thumb between the assembly and the conveyor frame requiring stitches.

While lowering a VFD (variable frequency drive) the injured and an other employee were going to bring down this 120 Lbs. unit from shoulder height. Injured employee had the heavy end on the unit and as it got to waist height the weight shifted to the side as they bent over. causing a pain to the right inguinal area.

While removing rocks from a crusher 1/30/2017 and welding in a crusher the next day employee reported a strain to right knee. On 2/16 ee was evaluated and the doctor prescribed Physical Therapy making the injury reportable. EE continues to work full duty.

After locking out the jaw crusher system, operator was attempting to remove a large flat rock from beneath the jaw crusher discharge chute when rock pinched his finger causing a laceration and broken tip of right pinky finger. He was wearing gloves.

The scaffold upright (vertical)supports have receiver cups that are welded by the manufacturer onto the upright poles at predetermined intervals. Thus it was slightly higher than the preferred level. Due to the construction, it would have been either be too low, causing employees to extend their reach too far, or too high, causing employees to be working very slightly bowed.

While working a steel shaft on a work bench, was moving the shaft into position. The work bench had holes on the table, the bolt nearest the hole fall-in while his left hand was under the shaft base which smashed his left middle finger at the first joint.

He alleges that on or about 10/20/2010 he was stacking type I cement and as he went to flip the bag to place into position, he felt a sting to the inside of his right elbow.

The IP needed to retrieve a piece of HDPU from the bellypan under a screen. The IP decided to slide down the side of the bellypan. When the IP slid down the side of the bellypan, his right foot struck the side when he reached the bottom. This caused pain in his right foot. The incident was not reported until 7/30. IP went to the DR on 7/30 and was diagnosed with 2 fractures.

While installing a pin on an conveyor slider bed, IP pryed up on the bed to line up the hole for the pin. As he was installing the pin, the bar holding the bed up slipped off, the pin shifted up, and caught the IP's first finger on his right hand between the bed and the pin. Injury required 3 stitches.

IP states that he was assisting clear material that bridged the inside of the clam shell gate that regulates the flow of material from a hopper onto a conveyor belt. IF lifted an extension pipe with his shoulder to open the gate. At this time the IP felt a small pain where the extension pipe contacted his left shoulder.

Employee allegies that on the 3rd of May he was stacking cement bags onto pallets on bagging process, at which time he claims that he strained his back. He did not have any pain untill May 23 at which time he reported to his supervisor. Late reporting.

IP was moving sawhorses in the welding shop. As the IP moved one of the sawhorses, a piece of angle iron fell from the sawhorse. The IP's foot was in the line of fire and was struck by the dropped object. The IP sustained a fracture to the little toe on his left foot.

Technician was installing torque converter which was on blocks, into a 769B off road dump vehicle. He placed straps under converter attempting to center it. When technician tried to push converter into position, strap slipped off to the side. Converter fell on technician's left foot causing fracture.

Aggregate filled the jaw to the level of the grizzly bars and the jaw stopped. The employee placed a rock on the pile on top of the grizzlies; the rock shifted and started to roll back into the jaw. He attempted to stop the rock and crushed his fingers between the rock and the steel plate between the jaw and the grizzlies.

Employee believes that the injury occurred while cleaning the plant at shutdown. When finished, IP stood up and felt a slight pain in the groin area. IP believed it was a muscle cramp that did not need medical treatment and the strain would subside with time. The pain subsided for a short period but returned. IP was taken to the doctor on 11/2 and placed on light duty.
