Showing all 31Employee was removing ladder from aggregate screening plant, got their finger pinched in hinge point on ladder. Laceration to index finger on right hand requiring stitches, no broken bones.
Miner was moving mobile metal welding bench and pinched hand/finger between bench and another piece of equipment.
Employee was assisting another employee with loading of truck tire into bed of pickup truck. Squatted down and lifted tire into back of pickup truck, straining groin in process.
Employee was hooking up screening plant to loader hitch. While placing pin in hitch, loader shifted forward, causing pin to tilt in hitch, resulting in employees fingers being pinched between hitch pin and counterweight of loader.
Employee was working on brakes on aggregate primary crusher (impact). While reinstalling brake drum, drum slipped off brake pads pitching finger between brake drum and axle.
Employee was assisting with tightening of bolts on crusher impeller bars. Bolts had been heated with torch in order to aid in process. Socket that was being used to tighten bolts became stuck on heated bolt. Employee grabbed socket resulting in 1st degree burns to two fingers due to heat transfer from hot bolt.
Stand pipe fell and hit employees shoulder.
Cleaning second deck screen in north screen deck by attempting to place roller on screen. Roller hit edge of screen box when EE was trying put it onto screen deck and it bounced back out smashing left fingers between roller and screen box.
Employee was trying to get the snow out of the dump truck box and pushing on the tailgate by hand. Did not get hand out of the way and smashed employee's right thumb in the tailgate.
Was pulling electric cords and hanging them in the rack on a bin. Thought that is was a pulled muscle. On 8/10 went to doctor and said do physical therapy. On 9/6 was placed on light duty because no progress. On 11/9 reached out of dump truck to shut lime bin off and got pain in shoulder. Had MRI same day and on 11/10 was diagnosed with left torn rotator cuff. Surgery 12/13.
Employee was laying down on the catwalk of the screen box tightening screen bolts. When employee went to stand up a sharp pain shot across lower back.
He was removing a dump truck tire from the tire rack. While pulling the tire out of the rack he felt a pull in his left shoulder. This was last day of work week and was decided to see how shoulder felt the following week. On 8/5/14 he was sent to occupational health because shoulder was still bothering him. He was placed on light duty. 8/12/14 recheck sent to therapy.
Folding up metal banding and cut left hand palm.
While filling the outside dual tire of a jo-dog jeep with air, he was pulling back to hold the air chuck on the valve stem when the air chuck came apart. The sudden release caused his shoulder pop as it flew backwards.

Was kneeling on knees pulling old belt out of bottom of leg #2 while the new belt was being pulled in above me. There was a piece of plywood that the new belt was laying on. When they pulled the new belt, the plywood slipped off of the beam it was resting on and hit him in the back of the head on his hard hat and snapped his head ahead.
Putting skirt boards on piggy back conveyor of surge bin. Was reaching out and lifting in an awkward position and pulled something in his back.
Went to exit cab of dump truck and pushed down on door handle. The handle broke, cutting his hand.
Repeated lifting of 50lbs sand bags at bagging operation.
Attempting to take the baffle off the 60' conveyor that was feeding the screen plant when the baffle slipped and cut the top of left hand.
Climbing down a ladder on primary crusher, fuel hose slipped down and the nozzel hit his left third finger. Did not seek medical attention, thought that it was brusied. On 1-22-08, smashed end of left third finger between shovel handle and wheelbarrow. Went to the doctor on 1-23-08 and has a tuft facture.
Stepping off of welding trailer and right ankle popped. States he stood up and right ankle popped again.
Attempting to hook the fuel wagon to a pick up, when the tongue fell off of pickup hitch and hit top of right foot.
TRYING TO GET A BELT RUNNING. A ROCK ROLLED OFF OF CONVEYOR BELT AND HIT HIM IN THE FOREHEAD.

THE DISSCONNECT SWITCH ON FRESH WATER PUMP HAD BEEN OPENED. HE WENT TO CLOSE THE BOX DOOR, BUT THE SAFETY LATCH WAS CORRODED. HE PUSHED UP ON THE SAFETY LATCH AND THE LATCH MOVED. THIS CAUSED HIM TO CUT FINGER ON LATCH OF DOOR.
HE PULLED A MUSCLE IN HIS BACK TAKING A GUARD OFF STACKING CONVEYOR.
FILLING WATER TRUCK WITH WATER. WENT TO MOVE HOSE TO FILL WATER TRUCK THAT WAS STUCK. PULLED ON 6" HOSE AND FELT A PULL IN LOWER RIGHT ABDOMEN.
WHEN USING RIGHT SHOULDER/ARM PAIN OCCURS IN RIGHT SHOULDER.
