Employees were on the ground laying a spreader bar down to begin break and as it was laying down EE had hand on the underside as opposed to on top and EE's first instinct was to push it and it pinched EE's hand between the spreader bar and saw horse smashing three fingers.

On April 3, 2025, at 3 PM in Arizona, an employee cutting a liner plate got it stuck. After securing and cutting it, the plate twisted, pinching their hand. They iced it, continued work, and later saw a doctor who diagnosed a left hand metatarsal fracture. Supervisor was notified; swelling reduced.

Employee working plant maintenance on feeder and tunnel when employee strained right shoulder. Employee notified manager and denied medical treatment at that time. Employee notified manager a second time (Aug 17, 2024) that the shoulder was still hurting, but again denied medical. On Aug 20, 2024 employee requested medical treatment and was placed on modified duty.

EE was operating the telehandler forklift picking up worn parts from VSI repair. EE parked the machine and picked up a casting part that had fallen off of the pallet. When EE picked up the part to place it back on the pallet EE noticed soreness in left shoulder.

Employee was cleaning the hydrated lime system. They pulled the rubber hose connected to the inductor, and the fitting broke off. The employee fell backward and hurt their lower back. The incident occurred on February 8, 2023; the employee refused medical treatment. Employee decided they wanted to see a physician on February 13, 2023, where modified duties were placed.

Operator was changing screens on the shaker deck in the morning and felt their back becoming inflamed from their body positioning. Employee was working through the pain operating the haul truck the remainder of the day when they noticed that the pain had become much worse. Was seen at clinic with a lower right back strain and right lumbar radiculopathy.

EE states that EE has had pain in EE's back and arms since august. EE has refused medical care on multiple occasions. EE contacted EE's supervisor on 2/3/2023, and requested to be seen for the pain. EE was seen at the urgent care facility on 2/6/2023. Work status report on file - regular work status approved by physician. EE returned to urgent care for follow up 2/16/2023.

A chunk of clinker became stuck in the bottom chute of a rail car. EE reached into the opening to free the clinker. When the clinker chunk came free and fell, it lacerated the middle finger on EE's left hand.

Employee 1 was working with employee 2 pulling wire through new conduit. The two employees took turns between pulling and feeding wire. Employee 1 eventually wrapped the mule tape around a piece of conduit to gain better leverage. After the work had been completed employee 1 told employee 2 that employee 1 believed employee 1 had tweaked back.

The crew was installing new parts in the clinker cooler. When EE felt sharp pain in my lower back. The following night shift there was tightness/tingling in Right leg and hip. After no improvement on night shift on 2/12/2022, EE reported it to safety at 5:30am, 2/13/2022. The object the employee was carrying was approximately 18 lbs.

Employee was working on cleaning a mixer at the conclusion of a job. The equipment was secured and properly deenergized, the employee then started to clean the tub and mixing blades of the mixer. As the employee was manually turning the mixing blades and removing material, EE's finger was pinched between the mixing blade and the tub causing a laceration.

The employee was inspecting a leaky radiator on a portable screen. The employee was sliding a steel plate off the engine compartment of a portable screen plant when the weight of the plate transferred down. The hand was on the plate that was lifted up. The finger got caught between the head pulley scraper and the metal plate.

EE was checking flow into KS1 when they got dust in their left eye. EE requested on 8/10/2020 to have their left eye evaluated. EE was diagnosed with a corneal abrasion of the left eye and given a prescription.

Employee was removing the bearing cap from a belt conveyer pulley for inspection. EE forgot to remove the tension from the counterweight and when EE removed the bearing cover bolts it shifted smashing EE's hand between the bearing cap and support structure. The result was multiple lacerations and fractures of the left ring finger.

IW was using a shovel to remove material from a lime stacker belt. A few hours after the task was completed, IW stated that their shoulder was hurting. IW was seen at the clinic and diagnosed with a sprained left shoulder joint. IW returned to work the following day on light duty.
