Injured EE was working underneath a 16M grader installing a pump. The pump splines into the transmission and the pump was still connected to its hoses. The EE's hands slipped and the pump swung downward on the hoses and the pump struck EE in the mouth and bruised his lip and broke his right eye tooth.

The employee was pulling drip line from the spool on the leach pad when a coworker pulled on the end of the line to stop the lines from reeling out. The abrupt stop caused the employee to feel a sharp pain in his lower back.

The double deck on the Gold Plant shaker screen had become loose and rotated from the carbon discharge chute. Employee tried to reposition the upper deck back into place. After about fifteen (15) minutes, he felt some pain in his groin area. Employee was able to complete his shift. He was seen by a physician's assistant the next day, and was diagnosed with a hernia.

EE WAS GETTING THE PREG POND LEVEL. HE WAS MOVING THE WEIGHT CONNECTED TO THE TAPE MEASURE. AS HE WAS DOING THAT, HE LOST HIS FOOTING AND TWISTED HIS ANKLE. FACTORS/REASONS: POND COVER IS A SLICK LINER; EE WAS NOT USING ANTI-SKID PADS THAT ARE STUCK TO THE POND COVER. LOST BALANCE; EE WAS NOT USING ROPE TIED TO TOP OF POND.
