Employee was inside LN12 primary mill performing lining. After moving a rock out of the way, employee stood back up and when doing so contacted a small shard of steel on the duckbill of the liner handler with forearm resulting in a laceration. Employee was seen by nurse and later received sutures at local hospital. Employee returned to work following treatment.

Showing 50 of 218Alleged: employee next shift stated to nurse, EE's back is sore, that EE lifted a box of bolts the previous day. No report was made to management on date of alleged claim. Employee has long-standing documented physical health matters.
EE was moving impact rollers from a pallet to a storage rack when one of the rollers moved rolling on top of left hand. EE pulled EE's hand out and felt a pop and pain in the wrist. Evaluated by physician and returned to work with restrictions.
EE participated in several consecutive shifts of manual labor (bucket brigade) requiring repetitive motion. Employee picked up plastic pail containing scraps (approx. 10lbs). EE states felt a pop in right shoulder and experienced immediate pain down the outside of upper arm. Employee was evaluated by two EMTs, and then transported to Bell for further evaluation. Restricted duty.
Employee was installing sheeting and hit elbow on a stud on a scaffold head while pulling a piece of sheeting. No pain at the time of incident but three days later it became swollen and very painful.
Mechanic reported lifting a battery(about 30#)out of a loader and felt "pop" in right shoulder. Mechanic initially declined medical assessment, but later provided conservative care instructions. After a follow up assessment, restrictions were ordered on 1-30-17. Mechanic reports having removed numerous batteries recently. X-ray report showed multiple chronic changes.
After shoveling concentrate for several hours employee c/o neck pain. Discomfort was not resolving with conservative treatment so PT was ordered. Employee completed a series of two treatments on 2-9-17.
Employee was trying to unplug lime piping under #2 lime silo. Lime dust contacted employee's face. Eyes and skin flushed. Brought to ER for evaluation. Prescribed eye drops. Had follow up appointment with eye doctor.
Employee felt strain in abdomen when removing a broken lifter from the #1 dryer exchange. It became known on 11-29-16 that the employee was diagnosed with an inguinal hernia. No work restrictions recommended.
Employee sustained laceration to right middle finger handling a warming oven. The employee was assisted by a partner in lowering the warming oven to the floor when the finger was pinched between the oven underside and floor. Employee was seen by site responders, and then brought to hospital where medical treatment was provided.
Employee reported strain in right shoulder after clean up work. Employee did not want assessment by site responder. Employee had surgery on shoulder 11-28-16.
Rolling rag and it caused a blister on toe. The blister caused toe nail to fall off and become infected. The employee is a diabetic. The accident report wasn't filed with the company until 2 weeks later due to the strange circumstances.
Employee dropped a bottle of 25% caustic soda and it splashed into eyes. Eyes flushed immediately. Taken to the emergency room. Seen by eye specialist the next morning. Given eye medications and returned to work.
Employee was working on changing 7B Filter stuffing box. When the employee was working to get the stuffing box from the grating level down to the work site, EE felt pain in lower back. Seen in the ER by MD and taken off work.
EE was measuring to cut a piece of steel. The piece of steel was on a wood pallet. The pallet gave way, and the piece of steel fell on the tip of EE's right middle finger causing a laceration. The EE was evaluated by a physician and sutures closed the wound. EE returned to work.
EE was chipping slag and stated he got debris in his left eye. Eye was flushed at the sight and was then evaluated at the hospital. He was given prescription eye medication and returned to work.
Two repairpersons were pulling the 3 CV head chute pan when it became bound on the bottom of the head chute & caused the lifting strap to break. The pan swung contacting the ee on left arm causing a laceration. He was evaluated by a Physician and sutures closed the wound. He returned to work.
EE was attempting to lift one end of wooden cribbing. EE felt discomfort to left arm once the cribbing was just being lifting off of the floor. The ee was evaluated by a Physician and given restrictions that were unable to be accommodated. Surgery is scheduled for 11-12-15.
An ee was standing nearby a pump transferring Sodium Hypochlorite 12.5% (bleach) from a bulk tote into boiler water profilers when discharge hose disconnected from pump, releasing chemical. EE was sprayed, resulting in chemical making contact with both eyes. Immediate flushing began and he was then evaluated by an Eye MD and given prescription eye medications.
Employee was installing a low head feeder deflector and got debris in the left eye. The ee had an evaluation with an Eye Specialist, debris was removed and prescription eye medication was ordered. He returned to work.
Employee lifted a precipitator launder cover up and set back on its hinges, while looking inside the launder the cover started to close hitting employee in side of face resulting in a small laceration requiring three stitches. Employee was treated and returned to work.
When the employee picked up one side of a temporary platform one of the 4x4 stiffeners came loose on one end causing it to swing into the employee's finger; laceration to the left index finger - 4 stitches.
While sorting side castings for T1 grate worker felt pain in right side lower back and hip
A welder was working on removing damaged handrail pockets when he cut his left middle finger on a sharp burr on the backside of the pocket. He was wearing proper PPE. He was seen by an EMT and taken to the hospital for further evaluation where 5 sutures closed the wound; the ee returned to work.
EE was putting dirty filter sectors from a pallet into the sector washer. When doing this, the operator felt a sharp pain in her back. EE was brought to the hospital for an evaluation and at which time she was taken off of work for one shift. She returned to full duty 1-12-15
EE was setting down a 4 inch steel pipe behind 8A filter. The pipe slipped and pinched his right index finger against the grating. He was evaluated by a Physician and a cautery pen was used to drain the hematoma under his nail bed. He was released to full duty at this evaluation.
EE reported an injury on 9-9-14 indicating that he had something in his eye and was alleging this to be related to work activities on 9-8-14. He was evaluated by an Ophthalmologist, a FB was removed and he was prescribed antibiotic eye drops.
Employee reported lower back pain while pulling a cable. He was evaluated by a physician and released back to work with restrictions.
EE was cleaning a low speed coupling cover when he felt a pain in his lower back. He was evaluated by a Physician after the event and taken off of work.
EE was installing a guard on Line 12 when dirt fell from above entering into his left eye. He and an EMT flushed the eye with no relief so was seen at an emergency room where he was evaluated & prescribed prescription eye medication. He returned to work full duty.
EE was lifting a return roller (40 lb). The ee picked up the roller by squatting down holding it near his body & bending at the knees. When he stood up, he felt a pain in left shoulder/bicep area. He was evaluated by a Physician on 6-19-14 and given a return to work with RESTRICTIONS.
EE was tasked to open a hinged access door. After the 4 pinch style clamps securing the door were removed, the EE started to open the door at which time the hinges broke and the door fell approx 3' pulling on right arm and shoulder. The EE was evaluated by a Physician and surgery was performed on 10-29-14.
EE was unloading a piece of steel (2x6x50) off the forklift & when she pulled the bar off the forks, her fingers got caught between the bar and the toe board. She was evaluated by a Physician and diagnosed with a fracture to her 3rd & 4th finger. She was taken off of work.
EE was working on Unit 2 repair when a casting fell off where the ee had his left hand placed resulting in getting his left middle finger pinched/laceration. He was evaluated by a Physician where he had the laceration repaired with sutures. He was released to return to work with restrictions.
EE was attempting to straighten the worm gear drive that was just placed on a pallet. While attempting to do this, he felt discomfort in his L wrist. He was evaluated by a Physician, a splint was ordered and he was given restrictions.
EE was removing the west guard for head pulley on line 7 feeder belt. He removed all lower belts & did not notice that the top was not secured. The guard fell on his head while removing it. He has been seeing a Physician with a conservative treatment plan & PT was ordered on 4-24-14. On 5-5-14, he completed a series of sessions.
2 ee were positioning a steel plate. The plate was 1/4" x 4' X 10'. 1 ee on each side of plate were positioning piece on top of structural steel frame work. The plate shifted & slipped falling to the structural steel. The inj ee was holding the plate when it slipped pinching his ring & middle finger of his left hand between the plate & structural steel.
On 1/8/14 the employee was coiling a cable up into a pile when he felt a sharp pain in his right calf. He was evaluated by a physican and released back to work with restrictions.
EE states he was pulling a hose down the east side of 3 conveyor walkway on grinding line 11. While doing this, he felt a strain in his back. He was evaluated by a Physician and taken off of work on 12-16-13.
EE was given the task to install a new pump. When he was searching for a bucket to dispose of broken grout he ducked his head to go under 2 low hanging rubber hoses. He raised his head, it contacted a stem of a gate valve resulting in him jarring his neck. He evaluated by a Physician and taken off of work for his next scheduled shift. Returned on 12-13-13 full duty.
The ee was moving a 4 x 4 x 10 piece of tubular metal. While attempting to place it next to a spare cobber drum, the magnet on the drum pulled the tubular piece to it. The ee's left thumb was caught between the 2 pieces of metal. He was evaluated by a Physician: the thumb was fractured and there was a suturable laceration. He was released back to work with restrictions.
While removing roller set, it slipped between the conveyor frame and pinched middle finger of right hand. Finger received 8 stitches.
Employee felt a "pop" in his lower back while lifting a 20 pound box. He was evaluated by a physician and physical therapy was prescribed. He attended his first session of physical therapy on 7/18 and his second on 7/23.
The employee was installing a trougher assembly on 3 conveyer line 3 and his left pinky finger got pinched against another trougher assembly while in the upright position. He was evaluated by a Physician and diagnosed with a fracture which subsequently led to restricted duty.
EE reported to the maintenance office with alleged shoulder pain 3-20-13, claiming it was from the previous day 3-19-13 while inserting clips into a conveyer belt. Stated that he did not feel any pain on 3-19-13 but his shoulder was sore upon waking up the morning of 3-20-13. He was evaluated by a Physician on 3-20-13 and given restrictions and returned to work.
Employee allegedly reported of right forearm pain when connecting a water hose to fill a water tank. He was evaluated by a physician on 3/16/13 and taken off of work.
Employee felt pain in left shoulder when lifting a pail of scrap material around #2 crusher. He continued to work and was evaluated by a Physician a week after the event. He continued to treat with a Physician, work full duty, and physical therapy was ordered as well as a cortisone injection. The employee has now completed a series of PT sessions.
Employee was shoveling snow to gain safe access. The top layer of snow was light and fluffy; as he got closer to ground level it was hard and icy. As he got closer to the ground and hit the ice and attempted to move this he allegedly strained his lower back. He was evaluated by a Physican and taken off of work 2-22-13; he returned on his next shift (2-25-13) with restrictions.
While raising his left arm to open the driver's door of a run truck, the employee allegedly felt his shoulder "pop out". He was evaluated by a physician on 1/26/13 and taken off of work.
Employee felt right shoulder discomfort after repositioning a 100 lb steel box on a tool bench. He continued to work with no further complaints. He saw a Physician on March 7th after having complaints and physical therapy was ordered. He began PT on 3-11 & completed a series of sessions.
