EE was in the laydown yard, cleaning trash and moving railroad ties. EE alledged he hurt his back while moving ties by hand. EE did not seek medical help for 2 weeks. EE's doctor recommended days away from work starting 10/25/2007. EE's doctor advised he had a possible herniated disk in lower back.
Employee was cleaning parts of disassembled grinder when he cut his finger on a piece of sharp metal. (Required stitches.)

Employee was one of five employees pulling on a 5" concrete pump truck hose when his right foot became entangled in a horizontal rebar mat. When the other employees tugged on the hose to move it, the hose was pulled across the top of his right foot and pushed the back of his right heel up against the rebar causing a strain to the Achilles tendon of his right foot.
Lifting liner section in ball mill with another ee, the injured party had lifted two other sections prior to this one. While lifting the third section he felt his right shoulder "give out".

EMPLOYEE WAS REMOVING DRAG CHAIN FROM CLINKER COOLER WHEN HE FELT PAIN IN HIS SHOULDER. INJURY DID NOT BECOME REPORTABLE UNTIL 2/27/04 WHEN HE BEGAN TO MISS WORK DUE TO SURGERY.
INJURED WAS USING DRIFT PIN TO DRIVE OUT TOP PINOF HYDRAULIC LIFT CYLINDER FOR BED ON 55-TON TRUCK. AS PIN DROVE OUT, CYLINDER DROPPED DOWN CAUSING THE DRIFT PIN TO PINCH THE INJURED'S FINGER AGAINST THE MOUNT BRACKET FOR THE CYLINDER.

WHILE TIGHTENING PIPE ON RAC-4 COMPRESSOR, INJURED'S HAND STRUCK A SHIM UNDER THE BASE OF THE COMP0RESSOR.

EMPLOYEE STEPPED UP ON A LADDER TO REMOVE A PIECE OF PLASTIC STICKING UP ABOVE THE DOOR ON THE IN-FEED HOPPER. HE HAS PLACED HIS RIGHT HAND ON THE LIP OF THE FRAMEWORK FOR THE SLIDING DOOR AND IT WAS PINCHED AS THE DOOR CLOSED.
INJURED WAS CUTTING LIDS OFF CLOSED TOP DRUMS WITH AIR CHISEL. AS HE WAS DOING SO, HE SCRAPED HIS FINGER ON SHARP EDGE OF DRUM LID. WAS NOT WEARING PROTECTIVE GLOVES PROVIDED BY THE COMPANY.
INJURED WAS REMOVING LIDS FROM METAL BUCKETS ANDDISPOSING OF THEM IN A TRASH HOPPER. AS HE DID SO, HE STRUCK HIS HAND ON A SHARP METAL EDGE SUSTAINING A CUT. HE HAD REMOVED PROTECTIVE GLOVES.
EMPLOYEE WAS CHANGING OUT A WATER BOTTLE IN THE WATER COOLER. (APPROX 43 POUNDS) AS HE LIFTED THE BOTTLE AND INSERTED INTO COOLER, HE NOTICED A PAIN IN THE GROIN AREA. DID NOT BECOME REPORTABL E UNTIL SURGERY ON MONDAY DEC 3, 2001.
INJURED WAS POSITIONING HAMMER SHAFT FOR CLEANING. AS HE DID SO, ONE END OF SHAFT SLIPPED FROM SUPPORT CAUSING HIM TO STRAIN HIS BACK AS HE CAUGHT IT.

EMPLOYEE WAS TROUBLESHOOTING THE PACKING AND LOADING PUMPMAN'S ALARM PANEL. THIS PANEL IS LOCATED IN AN ELECTRICAL ENCLOSURE AT THE VERY BOTTOM, ON THE BACK WALL OF THE ENCLOSURE. EMPLO YEE HAD TO BE ON HIS HANDS AND KNEES TO PERFORM THIS JOB. AT THE END OF THE JOB, EMPLOYEE COMPLAINED OF PAIN IN RIGHT KNEE. BECAME REPORTABLE ON9/10/01 WHEN HE STARTED TO MISS DUE TO SURGERY.
INJURED DROPPED NOSE PIECE OUT OF RESPIRATOR IN CHANGE ROOM WHILE GETTING DRESSED TO WORK. AS HE BENT OVER TO PICK UP THE NOSE PIECE, HE STRAINED HIS BACK.
EE WAS REMOVING A BOX FROM THE TOP OF A 4 DRAWERLATERAL FILE CABINET WHEN SHE STRAINED HER SHOULDER. DID NOT BECOME REPORTABLE UNTIL 12/3/01 WHEN INJURED STARTED MISSING WORK DUE TO SURGERY.
INJURED FELT PAIN IN GROIN AREA WHILE WORKING ONFAC #1 MOTOR. HE HAD PREVIOUSLY REPORTED PULL IN SAME AREA ON 8-14-00 WHILE REMOVING BACKUP POWER SUPPLY FROM TOP OF PLC CABINET IN KDC5 MOTOR CONTROL CENTER. DID NOT SEEK MEDICAL TREATMENT UNTIL 1-9-01. DID NOT BECOME REPORTABLE UNTIL 3-15-01 WHEN EMPLOYEE STARTED TO MISS WORK DUE TO SURGERY.
INJURED WAS POWER WASHING DRUM CLAMP ARMS FOR THE HYDROPULPER. AS HE BENT OVER TO MOVE THE PARTS TO WASH THE OTHER SIDE, HE STRAINED HIS BACK.
EMPLOYEE WAS RETURNING MIXER BLADE FOR INSTALLATION WHEN BLADE SLIPPED OUT OF HIS HAND, HITTING THE TOP OF HIS FOOT. BLADES HAD BEEN REMOVED AND REPAIRED IN MAINTENANCE SHOP, THEN TRANSPORTED TO TANK IN TRUCK. AS EMPLOYEE WAS SETTING BLADE DOWN, IT SLIPPED FALLING ONTO HIS FOOT.
INJURED HAD CUT LID OFF A CLOSED TOP DRUM OF MATERIAL TO REMOVE LIQUIDS. AS HE ATTEMPTED TO TIP DRUM, HE CUT HIS THUMB ON THE SHARP EDGE.
INJURED WAS MOVING A 55 GAL DRUM OF WASTE MATERIAL TO DIFFERENT ROW FOR STORAGE WHEN HE SPRAINED HIS KNEE. (DRUM CART WAS AVAILABLE). DID NOT BECOME REPORTABLE UNTIL HE STARTED MISSING WORK DUE TO SURGERY ON 8-3-00.
