Employee was routinely checking the bin capacity of the stone bin on the plant. He opened the overhead door (chain pulley) then stuck his head under the door to look in the bin. He accidentally let go of the chain and the door came down on his head.

EE WAS WORKING ON A LOADER WHEN HE PULLED ON THE AIRHOSE HE WAS USING WHICH WAS CAUGHT ON A CONCRETE TRUCK CHUTE, RESULTING IN THE CHUTE FALLING ON HIS BIG TOE.
