Two Miners were attempting to change a bit on a Jack Leg, when one of the miners put his hand between the bit and rib smashing his finger. Miner was taken to hospital, with Laceration and Finger fracture. Miner received stitches and given Cephalexin and Oxycodone and sent home.

Miner was hit on the head while installing utilities. Due to bad weather, ambulance could not arrive he was flown out to hospital for precaution. Miner was released that night after observation and x-ray showed no broken bones or need for stitches and has no restrictions from dr. This accident then qualifies for First Aid treatment.

Miner was exiting the DUX haul truck, when the cab door slammed shut smashing his left index finger. Miner was given first aid at the mine site, and taken to the clinic for treatment. Diagnostic Results- Tip of left index finger fracture and lacerated. Miner was given "cephalexin 500mg". Miner went back to work with no restrictions and no lost time.

Miner was drilling in the 5400 S2 stope with jackleg. Miner lost control of jackleg and his finger was pinched between the throttle control and valve body of jack leg. Miner received first aide but refused to see a dr. Feb 20, 2012 miner did go in to dr. Dr evaluation had fracture of third fingers.

Two miners were bolting in N1 area. Miner operating jackleg accidentally struck second miner on left foot. Injured miner did not report injury that night. Miner injured called mine office next day. Miner said he needed to go to Dr. Miner did not bring any information from Dr until return to work on Feb 21. He only brought release form not initial eval.

miner was rock bolting when the mat fell striking him on the left shoulder.They cleaned and bandaged the small cut. The next day(Dayoff)he went to the doctor the cut required two stiches.

EE said he and another worker were measuring a water line when he felt his right knee trying to lock up on him. That afternoon the same thing happen again. They did not report this to the supervisor. The next day on his regular day off, he said his knee was still sore and he put a brace and ice on it. 4:30 PM they took him to the hosiptal to get it checked out.

EE WAS CHANGING OUT A ROTARY AIRLOCK VALVE WHEN HE USED HIS HAND TO WIPE AWAY SWEAT FROM HIS RIGHT EYE. AFTER DOING SO HE NOTICED HE HAD SOMETHING IN HIS EYE CAUSING SOME IRRITATION. HE COMPLE TED HIS SHIFT & TRIED TO REMOVE THE PARTICLE AT HOME UNSUCCESSFULLY. THE FOLLOWING MORINING AT WORK HE REQUESTED A PHYSICIAN TO HAVE IT REMOVED. AFTERWARD HE WAS RELEASED TO WORK.
