Employee was in the process of changing out some fasteners on conveyor belt, when they pulled out wooden board they felt lower back pain.
While moving pipe for hopper support, the pipe clamp came loose causing the pipe to fall. Upon top end of pipe hitting the ground. The bottom end rose into the air, striking employee in the face. Flipped the employee and they landed on their stomach.
Employee was installing pin in hydraulic cylinder on John Deere excavator, EE stuck finger in there to make sure everything was lined up correctly. When cylinder was picked up, it caught EE's right index finger.
