The employee states he was moving a compressed gas bottle and strained his back. He continued to work the remainder of the shift and his next shift. On Sunday, February 6th he went to the local hospital and was diagnosed with a lumbar strain and was placed on two days of restricted work activity.

EMPLOYEE WAS REPAIRING THE LINER IN THE CHUTE. HE WAS HANDED A NEW LINER TO PLACE IN THE CHUTE AND TWISTED HIS BODY AND FELT PAIN IN HIS LEFT KNEE. EMPLOYEE WAS OFF WORK FROM 2/13/04 AND RETU RNED ON 2/16/04. EMPLOYEE DID NOT REPORT IT TO HIS SUPERVISOR UNTIL 2/16/04.

EMPLOYEE WAS UNLOADING STEEL BARS AND WHEN THE OVERHANG OF THE BARS DROPPED, EE REACHED TO CATCHTHE BARS AND FELT STRAIN IN HIS RIGHT ARM. EE DID NOT WANT TO GO TO THE DOCTOR AT THE TIME BUT HE CONTINUED TO HAVE PROBLEMS WITH HIS ARM AND DECIDED TO GET TREATMENT ON 10-7-02.

EE WAS CLEANING OFF THE RETURN ROLLERS OF THE CONVEYOR WHEN HE GOT SOME FOREIGN MATERIAL IN HIS LEFT EYE, UNDER THE EYE LID. HIS EYE WAS CLEANED OUT AND HE WAS GIVEN A PRESCRIPTION.
