EE was assisting in the removal of a pillow block on conveyor C0, at the primary crusher. After removing the block his foreman rested the block on a support frame to readjust hand position. The block fell and hit ee hand which was resting on another support. EE was sent to the Urgent care where he received six stiches to his right ring finger.

Showing all 5EE was cleaning/removing debris and rock that had become lodged between the frame and belt for the return for conveyor belt C8. The belt was not running, and was locked/tagged out. While removing the debris a rock became dislodged and smashed his right hand against the frame, fracturing his middle finger. No other injuries were present.
EE was changing screen decks that are held in place by J bolts. He was in the process of putting a nut on the J bolt when he cut his finger we took him to urgent care were he received stitches and was sent back to work. The doctor said there was no need for lost work.
EE was removing dozer tracks and his index finger got smashed in the process.
ON 2/25/05 EE ALLEGES THAT WHILE ATTEMPTING TO LOAD A BLAST HOLE WITH A BULK MATERIAL, EE FEET DROPPED INTO A VOID THAT WAS CAUSED BY SOLUTION MINING OF OVERBURDEN MATERAIL. EE BODY ALLEGEDLY TWISTED IN A MANNER WHICH THE DOCTOR FEELS STRAINED SCIATIC NERVE. EE WAS PLACED ON PAIN AND NERVE MEDICATION AND DUTIES WERE MODIFIED
