The shift crew was changing sorting screens on a sorting bed. The injured employee was guiding the suspended screen to a storage stack. As ee pulled on the suspended screen groin was caught between the screen and the stack. EE received painful contusions to groin and missed two days of work. EE has returned to full duties.

The injured employee was part of a team replacing a sizing screen on a Symons screening machine. While moving the replacement screen into place the edge of the screen caught on a machine guard. The employee pulled on the guard to release the screen and left hand was pinched between the screen frame and the guard. Employee received a laceration that required stitches.

The injured person was installing a bucket on a wheel loader. He was removing shipping bracket from the dog bone link. When bolt attaching the bracket bracket was loosened, the dog bone link fell and hit EE's shoulder.

The injured person was assisting with repairs on the crushing plant. He was helping to remove and carry a bearing for a shaker. The injury was not noticed at the time. He reported a sore back when he reported for his work shift on 12/30/10.

The injured employee was cleaning next to a conveyor by shoveling material into a bucket and then dumping the bucket on to the belt. The belt was not running. As he dumped a bucketfull on to the belt he felt a pain in his knee. The following day, the employee visited a doctor and he was advised not to work for at least a week.

The injured employee was regularly cleaning vibratory screens in the crushing plant by pounding the screen cloth with a metal bar. He developed a blister on his right index finger. The blister got infected and needed medical treatment.

When lifting cases of drinking water the injured employee felt a minor irritation in his back. Initially he did not feel pain or discomfort. Pain in his lower back progressively got worse until on 10/4/08 when he went to see a doctor. He has had difficulty sitting or standing for any length of time.

The injured employee was liftng a wear part from a VSI crusher. When he went to set it down he caught his right pointer finger between the part and another wear part that he was setting the part down on. His finger needed stitches and removal of the fingernail.

The injured employee was putting away his tools when he felt a pain in his lower back. Prior to this he had been replacing a conveyor idler on the crushing plant. The following morning, he was in more pain and visited the doctor. The employee has a history of back problems.

The injured employee was moving rocks at a crushing plant. A rock rolled off where he was stacking them and impacted his right hand. Resulted in lacerations to his fingers, one of which required sutures.

The injured employee and another employee were vchanging wear parts on a vertical shaft impact rock crusher. After finishing this task he continued working as the plant operator. He experienced some discomfort a while later, but continued working. He developed back pains and was unable to work the following day.

The injured employee was repairing the fuel system on an excavator. A fitting broke causing fuel to spill. While trying to contain the leak, diesel fuel splashed in his eyes. He flushed his eyes, visited a physician and got prescription eye drops.

THE INJURED EMPLOYEE WAS ASSISTING WITH UNLOADING 50 LB. BAGS OF ANFO INTO A STORAGE MAGAZINE. THERE WAS NO SPECIFIC INCIDENT THAT CAUSED THIS INJURY. HE DEVELOPED A SORE LOWER BACK AFTER COMPLETION OF THE TASK. HE HAD A PRE-EXISTING BACK CONDITION BEFORE THIS INJURY.

EE WAS ATTEMPTING TO REMOVE A WHEEL CHOCK FROM THE TIRE OF A ROCK HAUL TRUCK. HE STRAINED HIS BACK WHILE BENDING OVER TO PULL THE CHOCK OUT. THE CHOCK WAS WEDGED BETWEEN THE TIRE AND THE GRO UND.

DURING INSTALLATION OF 6" CHANNEL IRON STEEL MEMBERS, ONE SECTION SLIPPED OFF RESTRAINING CLAMP. WHEN IT FELL EE ATTEMPTED TO CATCH IT, CAUSING ONE END TO CONTACT HIM ON THE CHIN THIS CAUSED A LACERATION WHICH WAS TREATED AND REQUIRED STITCHES.

EE WAS HELPING TO LOAD A HYDRAULIC HAMMER BIT INTO A PICKUP TRUCK BED. THE EE'S LEFT RING FINGER GOT PINCHED BETWEEN THE HAMMER BIT AND THE FLOOR OF THE PICKUP, INJURING HIS FINGER.
