Emp was adding DEF to tank on Portable water pump in Quarry. As emp was removing the nozzle from the tank, emp felt a pop and sharp pain in right knee. Emp felt the knee and thought emp dislocated knee. Emp stated that oddly, emp didn't think emp moved in any direction to cause such an issue. Emp called supervisor. Sent to clinic. Now light duty

employee was removing the head nut from a cylinder as the nut came loose, it dropped on the workbench pinching the employees distal phalanx of the left middle finger between the bench and the head nut resulting in a minor fracture of the finger tip.
Twisted quickly while carrying 50 lb. bagged ANFO resulting in sprained spine.
Employee was taking a cutting edge off a pallet, and the cutting edge slipped. The employee did not move left hand out of the way fast enough, and it was pinched between the cutting edge and the floor. The employee waited until the next day to seek treatment. It was determined that the tip of middle finger was fractured.
Employee was draining water lines and blowing air through water line. All the air had not completely run out of the hose with additional pressure remaining and proceeded to hit the hand of the employee causing a contusion to the right hand.
Employee cut his finger with an old screen during the process of removing it. He received stitches and the tip of the left index finger is broken. Employee is a plant maintenance person working the night shift; after treatment was given, a return to duty with no restrictions was put in the status summary.

Employee waited till 5 PM to tell his supervisor what had happened. Employee was sent to a medical faculty for evaluation. He was told that he had a contusion on his left thigh.
Employee is scale clerk who was removing some vegetation from around a CCTV camera at the scale. Employee did not see how close he was to the guardrail and struck his face against the rail causing a laceration to his face
Employee was cleaning debris from catwalk when he felt a pop in back. Then mentioned he may have caused this injury previously on June 21st at 9:00 AM employee was moving parts and supplies into the new shop addition when he was walking backwards and tripped over a pallet, falling on his back. He did not report at the time. Employee returned to work with restrictions.
Miner was dismounting from the back of his service truck when his wedding ring got caught in a hook. His wedding ring cut into the meat of his ring finger causing a laceration. Employee needed 4 stitches to close the wound.
Employee was knocking grit off rails to install rubber components for a bucket. A piece of grit went into employee's right eye. Employee did not inform operator of injury until employee went to hospital to have grit removed from eye when eye became irritated. Employee suffered corneal abrasions.
Employee was removing the components of a flop gate at the discharge of a secondary screen. Supporting pin moved causing the flop gate to drop 6 to 8 inches. Employee attempted to catch the piece pinching it between the gate and side of a chute.
Miner was cleaning up a pile of build-up at a catwalk next to primary jaw crusher using a shovel. Employee felt twinge in his back that continued to worsen as shift progressed. Employee reported at next scheduled shift with severe pain. Employee suffered strained back, was given medication and therapy with lifting restrictions. Employee continued to work with restrictions.
Employee is a quarry QA/QC technician. Employee was shoveling and picking up buckets of stone when he began suffering chest pains. Employee thought it was heart attack and left site to report to ER. Was diagnosed instead with a strain of the intracostal muscles of the right ribs. Employee missed next scheduled shift due to injury and later placed on modified duty.
Employee is second shift maintenance. Employee alleges that while attempting to lift a 4x4 screen panel (wire cloth) onto a top-rail of a standard guardrail system, he sprained his left wrist. The employee states that he was attempting to toss the screens down to the ground below.

EE was opening garage door first thing in the morning. When metal dust particles came off of folding ledges that area part of garage door although the particle did not go into his eye the irratation made him rub eye until it went in eye and needed to be removed.
EE had Helper Tighting the setting of the crusher. a small size stone intzaferred with the hypaolic ram used for turning the top of the crusher. He reacher through the safety cover to remove the stone between strokes. The cylomone came out while his fenders were there, crushing then between the ram and rep of top.
While installing R.F. mounted tire on wheel, the tire rim shifted and caught employee's finger between wheel and assembled tire.
Employee was washing down screens with a fire hose. an Air bubble came through the hose and the hose whipped out of his hands. Throughout the day, his middle finger on his right hand became more sore.
THERE WAS A WEDGED BOLT ON THE JAW CRUSHER. THE EMPLOYEE REACHED IN TO CHECK IF THE BOLT WAS BROKE AND HIS FINGER GOT PINCHED BETWEEN THE BOLT AND DUST COVER.
EMPLOYEE WAS WORKING ON STONE CRUSHER. HE WAS LIFTING A STEEL BEAM, IT SLIPPED AND FELL ON HIS FINGER.
EE WAS CHANGING SCREENS AT THE CRUSHING PLANT. THE EE SLIPPED AND THE SCREEN FELL ON HIM.
EE WAS ASSISTING TWO OTHER EMPLOYEES CHANGING SCREENS. SCREEN BECAUSE OF WEIGHT MUST BE SLID IN PLACE. WHILE SLIDING SCREEN HE GOT RIGHT THUMB CAUGHT - PINCHED IN SCREEN.
EE STACKING EXPANDED METAL. METAL SHIFTED CAUSING LACERATIONS TO LEFT THUMB.
EE WAS PRYING A STEEL PLATE THAT WAS WEDGED IN THE CRUSHER, THE STEEL PLATE GAVE WAY AND LANDED ON EE'S FINGER.
EE WAS MOVING A PIECE OF STEEL. HE TRIPPED WHILE WALKING AND THE STEEL FELL ON HIS FOOT.
WHILE USING A HIGH PRESSURE WATER HOSE WATER WASSHUT OFF CAUSING FLUCTUATING PRESSURE OF WATER THRU HOSE-EE STRUCK NECK AGAINST BERM UNDER CRUSHER HORSE PLAY WAS INVOLVED IN ANOTHER EE TURNING WATER OFF & ON
EE CLAIMS THAT WHILE USING PROPANE TORCH TO THAWOUT ICE & SNOW AT CRUSHER AREA 20PD TANK THAT WAS ABOVE HIM FOR TORCH FELL APPROX 8' STRIKING HIM ON HARD HAT KNOCKING HIS HARD HAT OFF OF HIS HEAD
