Showing all 6The morning of 9/20/2012 as employee was descending from his bulldozer he felt a sharp pain in his right foot. It is believed to be from a combination of descending too fast and him getting his foot twisted and stuck in the ladder while descending.
employee was cleaning work area when he lifted a 50 pound bag straining his lower back to the point where he could not walk with out being assisted. He was taken to pahrump nv to the hospital.
Miner caught left "pinkie" finger in door of extruder. Informed company on 09/09, but said injury was minor. On 09/14 management set up appointment with local clinic for 09/15. Miner returned to work with splint.
Employee bent down to turn on water valve located approximately one foot from the ground. Twisted or tweaked his back in the process. EE is currently off work pending further medical evaluation and release from multiple doctors restrictions.

IT WENT AWAY AFTER AWHILE SO HE CONTINUE D TO STACK THE BAGS MORE CAREFULLY. EE DID NOT REPORT ACCIDENT TO HIS SUPERVISOR UNTIL APPROX 11:00 AM 9/19/00. EE RELEASED AND WORKED WEEK OF 9/25/00. STATED ON 10/2/00 HIS BACK WAS HURTING.
