Employee was installing lock ring on a 29.5 x 29 water filled tire. As he was lifting the ring onto the wheel, the ring slipped from his gloves, slid down his leg and landed on the side of foot. Fracturing his baby toe.

Showing all 59/4 employee says he injured his elbow. He claims the injury occurred on 8/23 no exact time or witness and no one was advised on 8/23. The other employees are also unaware of the injury this case is under investigation has not been deemed work related at this point as there is too many discrepancies in the story!
Employee bent down to pick up a piece of metal when he stood up, made note of slight pain. He continued working making no other complaints and finished his work day. Came to work the next day, no complaint, finished work day. Third day he stated at 11:30 am he needed to leave for a doctor appointment for back pain. He left work at 11:30am.

EE SAID HE HURT HIS BACK LIFTING A CUTTING EDGE ON MONDAY, ON THURSDAY HE SAID HE HURT HIS BACK ON WEDNESDAY PRYING ON A ROCK FROM THE JAW OF THE CRUSHER.
