Employee was standing on an incline in the screen deck with a hammer to hit the screen deck to remove built up sand from the screen. When reaching out his left foot slipped and he reached out to grab the screen deck. When grabbing the deck he strained his back.

EE STATES THAT HE HAS A SORE BACK FROM SHOVELING. HE ALSO HAS A SKIN IRRITATION ON BOTH CHEEKS FROM THE ELASTIC BANDS ON HIS RESPIRATOR. HE CAME INTO WORK THE MORNING OF 08-14-2002 & COMPLAINE D OF A STOMACH ACHE & WENT HOME. THE NEXT DAY HE CAME INTO THE OFFICE & STATED THAT HE HAD A SORE BACK & THE IRRITATION ON HIS FACE.

EE WAS WALKING UNDER A CONVEYOR WHEN A LOOSE END OF HIS BANDANA WAS CAUGHT IN A PULLY AND HIS HEAD WAS PULLED BACK. THE BANDANA WAS WORN UNDER HIS CAP AND APPRENTLY WAS NOT TUCKED IN PROPERLY. THE PULLING ON THE BANDANA RESULTED IN A SORE NECK.
