The EE was improperly loading steel into a drill. They laid the boom down & were adding steel directly into the carousel. One end of a steel fell out while handling it. This caused the other end to fall out & smash their thumb between another steel. Upon seeing a doctor, it was diagnosed the the EE ruptured a tendon in their right thumb.

While changing a wear liner on the T-400 crusher, EE was struck in the arm by a wear plate that took an unexpected fall during removal.

While scooping a bucket of stemming rock EE's knee bent a little sideways and gave out and made a loud pop. EE sprained EE's right knee.

Employee poked self in the eye with ear-piece of safety glasses on 08/23/2021 while trying to put them back on, and requested medical treatment on 08/25/2021. Was treated with an antibiotic eye drop and released straight back to work. No restrictions.

Tech was flushing a cooling system on a loader. When draining the coolant EE removed the lower radiator hose and splashed EE's right lower arm and right neck with hot coolant. This caused second degree burns to EE's right arm.

Employee was replacing the AA batteries in the remote control unit for the tracked crusher. As EE flipped the first AA battery out, some liquid on the end of the battery hit EE on the side of the nose & above safety glasses. The liquid above the safety glasses fell down into EE's right eye & immediately started to cause irritation. EE was prescribed antibiotic eye drops.

While removing a guard from C1 conveyor the guard struck the miner on the left forearm causing bruising and swelling. Miner reported the incident but stated that they felt that they would be fine applying ice to the arm and did not want medical attention at that time. On 6/18/19 the miner reported to work and the swelling was worse so they ere taken to the Hospital ER.

While opening the flop gates on an Allis screen to control the rock overflow employee got the tip of the left ring finger below the fingernail but above the last knuckle closest to the end of the finger smashed off when the employee left hand testing below the flop gate handle

Finished changing hydraulic vibrator on pep-shaker. Putting away tools - 1/2 inch break-over bar slipped through hands & caught top of right foot resulting in small puncture wound and swelling.

EE WAS POURING CONCRETE AT THE QUARRY. HE HAD RUBBER BOOTS ON BUT SOME CONCRETE GOT ON THE BACK SIDE OF HIS LEGS.
