The mechanic was changing a scoop tire and in the process the tire fell on his mid section. The EE completed his shift and continued to work, on 9/10/08 the EE decided to see a DR. THe DR diagnosed a umbilical incarcerated hernia.

On 8/21/2008 this foreman was loading posts in a scoop bucket and he felt a pain in his lower back. The foreman continued to work and on 9/10/08 decided to see his physician. Therapy and rest was prescribed.

The employee was shoveling belt and felt a pain in his lower back. X-ray was negative and the employee returned to work. This employee notified management that additional back problems from the previous incident required him to return to the physician. Additional rest and treatment was prescribed. (Started missing 8/22/08).

The employee was assisting changing a shuttle car tire. The lug bolts were removed and the tire suddenly came off and rolled, striking the employee's right foot and lower leg. X-ray was negative, bruise.

The employee was lifting a cable reel on a roof bolt machine and strained his left wrist. The employee continued to work with some pain. Pain continued an an MRI was scheduled and the results showed a tear in the left wrist cartilage and tendon.

Injured did not feel any pain until the next morning in his lower back. He went to the doctor and had xrays and it showed he had inflammation in his lower back. He also went to a chiropractor who said he had 2 bones "out of wack".

EE was using a sledge hammer to knock down a solid block stopping and a block fell and struck his left hand. X-ray showed his two middle fingers were fractured.

EE was using a bottom belt roller to pound a christmas tree water valve on the end of the east water line. While pounding his thumb was pinched between belt roller and the fire valve. x-ray showed fracture to his thumb. EE did not return to work his next scheduled shift.

Pulling belt back together When they started pulling the belt back together with a sleve wheel and cable, the roof bolt broke that was attached to a belt hanger. The belt pulled backward about 3ft and the EE was struck above the right eye with the belt clamp. The EE required 7 stitches. Four days restricted duty.

EE WAS MOVING MINER & BENT OVER TO TOSS CABLE OUT OF THE WAY. AT THIS TIME HE FELT A PAIN IN HIS LEFT GROIN AREA. HE FINISHED HIS SHIFT & ON 9/5/05 PAIN INCREASED. 1ST DIAGNOSIS WAS A CYST & FOLLOW-UP EXAM 9/15/05 DETERMINED A CAT SCAN WAS NEEDED. CAT SCAN SCHEDULED 9/22/05. AT THIS TIME THE ACCIDENT IS CLASSIFIED AN OCCUPATIONAL ILLNESS.

RE-AGGRIVATION OF INJURY THAT HAPPENED ON 8/20/04 (COLLISION OF TWO MANTRIPS TRAVELING TO SECTION). ON 11/8/04 THE SUPERVISOR WAS SHOVELING BELT AND FELT PAIN IN HIS LOWER BACK. EXAM SHOWED A MUSCLE PULL TO SAME LOWER BACK AREA. THE SUPERVISOR WAS UNABLE TO RETURN TO WORK HIS NEXT SCHEDULED SHIFT.

The employee was operating the remote control miner and needed to throw the cable out of the roadway. When he bent over and tossed the miner cable out of the roadway he felt a pain in his lower back. The employee finished his shift but was unable to return to work his next scheduled shift. Muscle pull, rest and prescription for pain.

The employee with the assistance of 4 mechancs was changing a floating wheel axle on a s/c. There was no mention of an injury during the shift. After the shift the ee mentioned to a co-worker that his back was sore. He called the next day and stated that his back was sore and needed an examination. The physician gave him pills to relax his back muscle and 4 days away from work.

The employee was removing his drill steel from under a box of resin and a 3 ft bolt wrench was on top of the resin box. When he pulled the steel from under the box, the wrench fell and struck his right small finger. The tip of the finger was out of place and the physician pulled back in place.

THE EMPLOYEE WAS INSTALLING A TRACK ROLLER AT THE PIT-MOUTH FOR THE HOIST CABLE. THE EMPLOYEE WAS POSITIONING THE ROLLER IN PLACE AND THE ROLLER KICKED AND PINCHED HIS RIGHT INDEX FINGER. THE EMPLOYEE'S FINGER REQUIRED SIX STITCHES.

THE EE WAS LOADING A DRIVE SHAFT FOR A ROOF BOLTER IN THE BILL OF A SCOOP BUCKET. THE DRIVE SHAFT WAS JUST CHANGED AND HAD OIL AND GREASE ON IT. THE DRIVE SHAFT WAS IN A LOCATION WHERE IT NEEDED TO BE LIFTED IN THE BUCKET. WHEN THE EE LIFTED ONE END, IT SLIPPED OUT OF HIS HANDS AND LANDED ON HIS LEFT FOOT. E-RAY NEGATIVE, BRUISE.
