EE advised us letter dated 8/4/14, was received by employer on 8/8/14, illness to lower back is related to wear & tear from 13 year tenure at Greystone Quarryes, Inc. Attached copy of letter for reference. E-mail, ee advised us, as per his doctors communication and the x-ray results, the correct date for the injury/illness is 6/17/14. See attached copy of the email for reference

EE was pushing & positioning a rock on a roller conveyor into the head of a hydraulic stone splitter when EE heard a popping sound in L-shoulder. L-shoulder felt sore. EE did go to his doctor on occasion with no restrictions or missed work days however, upon advice of doctor EE just completed surgery on L-shoulder on 9/13/13 & will be out for undetermined time during rehab.

Pulling stone on roller feed conveyor attached to hydraulic stone splitter the employee shoulder started hurting. When moving shoulder pop could be heard. Possible sprain (r) shoulder. Stone was being pulled as normal operation on roller feed tray of splitter. At time of incident was not reported to upper management. Reported on 7/12/11. Off work effective 7/12/11.

EE was pushing a stone on a manual roller conveyor to feed a hydraulic stone splitter. While pushing the stone he felt pain in his groin area.

EE WAS PICKING A STONE OFF THE QUARRY FLOOR WHEN HE EXPERIENCED A SHARP PAIN IN HIS LEFT GROIN AREA. EE HAD HERNIA OPERATION SAME DAY AS INJURY AND RETURNED HOME 3/30/05. EE WILL SEE DOCTOR ON 4/7/05.

INJURED EE DID NOT REPORT INJUGY TO HIS SUPERVISOR ON THE DAY OR TIME OF INJURY. THE SUPERVISOR GIVEEPPE CATANIA ONLY BECAME P OF THE INJURY WHEN EE CALLED OFF WORK ON 3-21-02 ON THE DAY AFTER THE INJURY. EE WAS APPARENTLY HANDLING SOME HEAVY PIECES OF STONE AT THE STONE SPLITTER AND A STRAINED HIS BACK DURING SHIFT.
