Employee was adjusting the door on the feed hopper at the crusher. The door has a handle to lift to raise or lower door. As the employee lifted up on handle employee strained a muscle in employee's back.

Employee lifted pit truck hood and felt back pain in his back. He continued working for approximately one month at this time he went to the doctor on 07/16/2013. The doctor put employee on restricted work duty at this time. Because of the restricted work duty, we have been unable to accommodate this employee. He is still currently off of work.

Employee was working on a muffler with a hole in it. He had his finger near the hole when another employee kicked the muffler. Hole flexed and cut his left pinky fingertip with fingernail also.
