Mechanic was putting new belts on shaker conveyor. The shaker shaft is offset and the belt rolled back causing EE's finger to get caught between belt and sheave.

Maintenance Tech was given the task of cleaning the spill pans used to catch acid under Fork Truck batteries that were being charged. During the process of cleaning, EE's eye was irritated by fumes.

A Maintenance Tech was working with a partner to raise an inlet flange to the top of a screener (weight 49 lbs.) The piece shifted and one Maintenance Tech suddenly had all of the weight. EE experienced a back strain and returned to work with restrictions after medical evaluation.

The incident was not reportable until the employee asked to see a doctor on Sept. 7. The employee was removing dust covers from the top of a railcar and struck hardhat on the dust cover holder when the employee stood up. The diagnosis was a strained neck muscle. Employee was prescribed two medications & restricted from carrying or lifting more than 5 pounds until Sept. 12.

Contractor positioned a pane of glass in a window frame. The contractor was working below when the aforementioned pane of glass fell out of the window and fell on hardhat. A piece of broken glass landed on and cut the top of right hand.

During a mid-shift inspection of the work area, a hazard was noticed. The employee thought it to be a good idea to look further into the hazard to see if EE could fix it. Watching the kiln and assessing danger EE moved closer to the Gaurd to see if it needed removal or adjustment. EE's hand rested on the guard momentarily and the guard failed pinching EE's finger.

Two Maintenance Techs were working to replace screens in screener. Techs moved top ring section with lid (approx. 60 lb). One Tech felt sciatic pain in back and leg. Evaluated by occupational medical services, advised to return home, rest, not report for work, take a prescription muscle relaxer. Appointment made for orthopedic evaluation on Wednesday, January 17th.

Mechanic was lowering an extension ladder 135 inches to the ground from a safety railed platform with toe boards. The ladder began to slide down the rail when the bracket for keeping the extension ladder in one piece slid against left index finger. Mechanic was not wearing gloves during the incident. The wound required 8 stitches to close. There are no restrictions on work duty.

Mechanics were replacing the seed pipe to the # 7 Ball former on the 2nd floor of 426. Pipe rotated out of position. Mechanic tried to arrest the motion of the pipe and felt a pain in the lower back. Member given restrictions and Rx medications.

Operator was climbing a ladder on the 55 Building Charge Floor Hopper when employee felt a pop in the wrist. Employee stated it did not hurt so they did not report the incident to anyone. Upon going home and waking up the next day there was swelling and pain radiating down the wrist and thumb.

Operator was contacted with hot material from the waist up while checking dust flow from the cyclone to the leach tank. After opening the knife gate valve he opened the clay bailey to check the flow. Initially the dust flow was okay but then stopped. This was followed by a large slug of material that broke free and flushed out of the clay bailey, contacting the operator.

In the course of an outage a section of exhaust duct was removed from Kiln No. 1. During the refractory bake-out combustion gases were released into building 405. The next day a Calcines filter floor operator reported that he had a headache and became disoriented, and after going home and sleeping felt like he had a hangover.

Calcines employee was unloading chemicals on the 3rd floor Filter end. As he was pushing a dolly loaded with fluoride, he struck his right hand against the edge of a piece of sheet metal guarding that was wrapped around a pipe. The employee suffered a cut to his hand that was fairly deep that required stitches.

A employee was cleaning the magnet located on the Tabular 112 Fired Ball belt. The employee had to place his hand and arm between the belt and magnet to clean the magnet. The cable which holds up the magnet failed. This caused the frame of the magnet frame to fall on the employee left hand.

While recharging A-2 Mill, employee was in West access hole when access hole covering fell striking his hardhat. He refused medical evaulation until 7/3/07 where he was diagnosed with a contussion and cervical strain and released to work with restrictions.

Employee attempted to measure the hole pattern on a Vibrator by rolling it onto its side. The Vibrator was heavy and the Employee let it back down. His right hand - small finger became caught between base and pallet resulting in a laceration and inability to straighten finger. He was diagnosed with a contusion and closed fracture of the right small finger.

Employee retrieved Pannivis Filter Cloth from storeroom. He removed the Filter Cloth (128lbs 90"X10") from storage rack, and carried it outside to truck. He stated that he felt pain in his lower back when he maneuvered Filter Cloth through door and upon putting it into the truck. Employee did complete task of replacing Filter Cloth with assistance from 2nd Operator.

Injury Date of 5/24/2006, not reported until 6/6/2006. Incident occurred at #3 Bagging Station, employee had to manually remove sticking product bags weighing 50lbs from tube fill nozzle. Material was hot and would stick to metal nozzle, automatic equipment was not effective in removing bags, operator would reach down and out and pull bag off nozzle.

Employee was attempting to remove a drinking water container weighing approximately 41.5lbs from the storage bin, at approximately waist to chest high. He pulled the container from bin, and in a fluid motion moved container up to and rested it on his shoulder for transporting. After he did this motion he felt a sharp pain in his lower abdomen.

General Mechanic welded cracks around the mounting bracket of the No. 3 bin vibrator in Bldg 400. To access the vibrator he pulled himself up between 2 pipes. As he did he felt a pull in his left shoulder. He was seen by the company doctor 12/7/2005 & given a restriction from overhead work, climbing and will be seen by a physical therapist. Not reported until 12/06/2005.
