Ee was installing a pin. Pin fell about 18" onto Ee's left hand, causing lacerations to tips of ring and pinky fingers. Laceration to ring finger required 8 stitches to close the wound.
Employee was stepping down while steadying themselves with Left Hand on a vertical beam. As the hand slid down beam, a burr lacerated the tip of the Left Middle Finger. Requiring sutures to close wound.
EE was working in the auto shop, underneath a truck, when they felt something fall into Right Eye. EE rinsed the eye, but still felt symptoms. EE was seen at clinic, and Dr prescribed antibiotic for preventative measures and released to full duty.
Ee was assisting in changing of hoist guide brackets. Some loose salt contacted the Ee's upper left back, just below the shoulder causing a fracture in 3 ribs. The fractures was not found by x-ray, but were discovered during an MRI.
Ee was exchanging oxygen bottles and crushed the tip of Left Pinky Finger, causing damage to fingernail and slight fracture to very tip of pinky.
Lining up a pipe flange when HDPE pipe laid over and struck shoulder and pushed employee into side of excavation. Pain in back showed itself several days later.
Employee stepped onto cage, and updraft blew debris up past employee's face. Something went behind safety glasses and went into Right Eye, causing minor irritation. Employee flushed eye several times. Later in the day, eye was still irritated, so we took employee to town. Dr. flushed the eye. Dr. gave prescription for antibiotic ointment for precautionary measure.
Trolley had slipped off of an I-beam. Employee reached across to lift trolley back onto I-beam and felt a pain in stomach. It was determined that the employee will need surgery to repair a minor hernia.
Ee inspecting pump parts. As part was set back, Ee pinched finger against another part. Resulted in a laceration, requiring stitches.
EE lifting the travel-side return idler bracket after splicing conveyor belt. Idler shaft had not fully set in bracket & slipped out. Sudden release carried bracket to EE's face chipping a tooth--required dental repair.
EE reported feeling a pop in R shoulder while loading tools onto vehicle. Injury treated conservatively but subsequently determined to be a torn rotator cuff requiring surgical repair on 03-18-2016--first LWD.
EE attempting to close ball valve at 3-A preg cyclone. Significant force was required to close the valve resulting in contact with an adjacent (2-A) valve. A laceration to the R thumb required stitches to close.
EE pinched R #3 fingertip between conveyor take-up hyd. cylinder and frame. Stitches were required to close the wound.
EE was in the process of removing baffles from 2A Hot Leach tank. The baffle slipped and lacerated EE's left index finger. The EE was wearing leather gloves. The wound required stitches.
EE was assisting in removal of pin mixer bottom liner. The drum was blocked but allowed slight movement. EE reached into the enclosure to grip the liner when the drum bumped slightly pinching R index finger. Laceration required stitches.
EE was moving a section of water hose for transport. He felt a sharp pain in lower back. The injury was treated and resulted in LWD on 6/4
EE was helping guide the removal of a large equip part and was focused on clearance in one direction and not on the position of his R hand. The equipment shifted unexpectedly, pinching the R palm. Stitches required to close wound.
EE stepped back while co-worker struck link for conveyor chain to remove link. Steel chip from the link struck EE in face. Debriding attempted at ER. Stitches to close
EE was loading 12" pipe flanges onto a flatbed truck and cut his L #5 finger on flange. The wound required stitches.
EE strained her back while replacing a pump motor guard in #1 hoist house on 4/10. The injury was treated conservatively without restriction or LWD. Upon medical re-evaluation she was removed from active duty effective 4/23
EE was checking oil on Lowtrac as part of pre-op inspection when the engine cover fell striking EE's R forearm. The impact caused a fracture of the R Radius.
On 2/13 EE reported abdominal strain while loading belt idlers onto a trailer. EE was able to continue performing assigned duties pending medical evaluation. On 3/6, EE underwent outpatient hernia repair (1st lost workday).
EE assisting in attachment of 12" diameter driscopipe section. Pipe end shifted striking EE who fell to ground. Injury to R knee. RTW with Restrictiions
EE reports bleeding off air pressure before disconnecting compressed air hose. Despite this, hose end whipped striking EE's jaw. Stitches to lip, some dental damage. (filed in compliance with PPL: P13-V-02)
EE was rolling an oversize rock off conveyor and pinched R ring finger against conveyor. Laceration and fracture to tip of finger
EE manipulating starter motor to remove from D8 when motor slipped and fell striking the ground and then tipped toward the belly pan (on ground) pinching L#3 finger resulting in a laceration that required stitches.
EE assisting manual extraction of plug at 1st stage splitter box. EE felt lower back pain. EE self-treated until 6/23 and sought medical attention on 7-day break. On 6/27 EE missed scheduled work shift. Back strain complicated by compression of vertebrae.
EE was attempting to move a stored hydraulic cylinder from the T-tower. Partner moved one end and the cylinder slipped from the edge of a raised (12") surface. The end of the cylinder struck EE's L foot, resulting in a fracture. EE returned to modified work on the next scheduled shift.
EE manually removing chunks of product from L-3 Dome. Slipped on material and chunk fell striking R hand. Treated as FA incident. Subsequent medical evaluation revealed fracture that was treated by immobilization on 2/21. EE continues to perform normal load-out duties.
EE loading 42" bottom idlers into scoop bucket, pinched R thumb between idler and side edge of bucket. Lacerated tip (required stitches)
EE reported sore R knee on 10/12/13 attributed to using plant stairs on 10/11/13. Sore knee symptoms were treated conservatively and EE performed normal duties until arthroscopic surgery was performed for medial meniscus tear on 5/9/14--first lost-workday.
Employee strained his back while carrying grating and was released to full duty at his initial visit. At one of his follow-ups, 2 weeks later, the employee's work status was changed to restricted duty.
While removing the rear tire on a ram car, the tire shifted unexpectedly pinching the EE's R thumb. The injury resulted in medically treated fracture of the tip of the R thumb.
EE was installing rods in rod mill when a rod slipped from the EE's hands falling on L foot resulting in fracture to #1 metatarsal.
EE advancing power, hanging HV cable & nip--partner let go of cable, heavy load strained L Shoulder. RTW normal work until diagnostic arthroscopy performed--No damage found, however LWD followed procedure starting 12/03--indirectly related to injury.
EE installing gear case attempting to align bolt when case shifted pinching L index finger against washer resulting in laceration requiring stitches.
EE pulling a conveyor flight from belt tail and felt pain in lower back. First aid applied and returned to assigned duties. On 2/27, reported deteriorating condition--treating physician issued no-work order. LWD started 2/28/2012.
Received documentation 09/28/11. While setting a piece of pipe down on a flat bed, the tip of his left 3rd finger was caught between the pipe and flatbed, causing a fracture.
Employee removed PPE (glasses) for lunch and felt something in his left eye. He did not seek medical treatment until 9/1/11.
Truck driver was delivering tires to mine shaft #2, the tire struck an angle iron on lift gate. The tire deflected and struck driver causing him to feel a pinch in his lower back. Injury was not reported to employer until 9-6-11. He was having low back pain and was seen by doctor. The employee was treated as first aid until 9-26-2011 when he began losing time at work
Cutting large rolled plate (20'X10'X1/4") using a cutting torch to cut plate to fit through entry of Floc Tank, plate was sitting up right while being cut, after cut was complete plate shifted & started to fall forward, employee grabbed plate to stabilize & keep plate from falling against him, lowered to ground level with help and strained/wrenched his back in the process.
We inadvertently didn't include this project as MSHA on quarterly 7000-2. As per MSHA directive, we have amended the 7000-2 for the 2nd quarter 2011 & are also filing an initial 7000-1 for this reportable injury. EE was rigging a beam. After it was raised, wind blew & the beam began swinging. EEs L hand was where the connection was to be made causing FX to L middle & ring fingers.
EE experienced hernia while lifting a shuttle car tire to remove a chain. Treated as a first aid until surgery performed on 6/27/2011. Surgery was performed during EE scheduled days off.
Employee was preparing caps for dryer. While preparing, stainless steel metal rolled causing a cut to his right thumb. Immediately after the incident, supervisor took precaution and medical treatment was followed.
Small left finger became caught between a cable tray and frame when a co-worker pushed tray into 45 degree angle, as instructed by injured employee.
EE was tossing a coil of hose off his shoulder and lunged forward to clear the hose & felt pain in his abdomen -- also felt nauseated. Noticed a knot near his navel. Aggravated hernia.
EE was removing lug nut on Shuttle Car Wheel when the rim separated discharging air. EE was caught in the line of fire of the air discharge and debris - laceration to head required stitches.
Employees were removing the feed chute to replace screens in the west rotex screen. As employees were moving the feed chute, the weight shifted & EE's right hand was caught between the handrail & the chute flange. EE received a laceration & small chip fracture to the right ring finger
EE was using a bottle jack to help support and lift a detached rub rail of a miner. While jacking up, the rail, the jack moved. EE tried to hold the rail in place. The rail fell pinching EE left middle and ring finger against the ground resulting in a fracture of the tip of the L ring finger.
On 6/1/2010 EE Strained Right Shoulder lifting Load-Out By-Pass Gate Handle. First Aid Treatment was provided. Out Patient procedure to repair lesion on 11/12/2010, EE RTW next scheduled shift on 11/15/2010.
