Ee was installing a pin. Pin fell about 18" onto Ee's left hand, causing lacerations to tips of ring and pinky fingers. Laceration to ring finger required 8 stitches to close the wound.

Employee was stepping down while steadying themselves with Left Hand on a vertical beam. As the hand slid down beam, a burr lacerated the tip of the Left Middle Finger. Requiring sutures to close wound.

EE was working in the auto shop, underneath a truck, when they felt something fall into Right Eye. EE rinsed the eye, but still felt symptoms. EE was seen at clinic, and Dr prescribed antibiotic for preventative measures and released to full duty.

Ee was assisting in changing of hoist guide brackets. Some loose salt contacted the Ee's upper left back, just below the shoulder causing a fracture in 3 ribs. The fractures was not found by x-ray, but were discovered during an MRI.

Employee stepped onto cage, and updraft blew debris up past employee's face. Something went behind safety glasses and went into Right Eye, causing minor irritation. Employee flushed eye several times. Later in the day, eye was still irritated, so we took employee to town. Dr. flushed the eye. Dr. gave prescription for antibiotic ointment for precautionary measure.

Trolley had slipped off of an I-beam. Employee reached across to lift trolley back onto I-beam and felt a pain in stomach. It was determined that the employee will need surgery to repair a minor hernia.

EE lifting the travel-side return idler bracket after splicing conveyor belt. Idler shaft had not fully set in bracket & slipped out. Sudden release carried bracket to EE's face chipping a tooth--required dental repair.

EE reported feeling a pop in R shoulder while loading tools onto vehicle. Injury treated conservatively but subsequently determined to be a torn rotator cuff requiring surgical repair on 03-18-2016--first LWD.

EE attempting to close ball valve at 3-A preg cyclone. Significant force was required to close the valve resulting in contact with an adjacent (2-A) valve. A laceration to the R thumb required stitches to close.

EE was in the process of removing baffles from 2A Hot Leach tank. The baffle slipped and lacerated EE's left index finger. The EE was wearing leather gloves. The wound required stitches.

EE was assisting in removal of pin mixer bottom liner. The drum was blocked but allowed slight movement. EE reached into the enclosure to grip the liner when the drum bumped slightly pinching R index finger. Laceration required stitches.

EE was helping guide the removal of a large equip part and was focused on clearance in one direction and not on the position of his R hand. The equipment shifted unexpectedly, pinching the R palm. Stitches required to close wound.

EE was checking oil on Lowtrac as part of pre-op inspection when the engine cover fell striking EE's R forearm. The impact caused a fracture of the R Radius.

On 2/13 EE reported abdominal strain while loading belt idlers onto a trailer. EE was able to continue performing assigned duties pending medical evaluation. On 3/6, EE underwent outpatient hernia repair (1st lost workday).

EE was attempting to move a stored hydraulic cylinder from the T-tower. Partner moved one end and the cylinder slipped from the edge of a raised (12") surface. The end of the cylinder struck EE's L foot, resulting in a fracture. EE returned to modified work on the next scheduled shift.

EE manually removing chunks of product from L-3 Dome. Slipped on material and chunk fell striking R hand. Treated as FA incident. Subsequent medical evaluation revealed fracture that was treated by immobilization on 2/21. EE continues to perform normal load-out duties.

EE reported sore R knee on 10/12/13 attributed to using plant stairs on 10/11/13. Sore knee symptoms were treated conservatively and EE performed normal duties until arthroscopic surgery was performed for medial meniscus tear on 5/9/14--first lost-workday.

Truck driver was delivering tires to mine shaft #2, the tire struck an angle iron on lift gate. The tire deflected and struck driver causing him to feel a pinch in his lower back. Injury was not reported to employer until 9-6-11. He was having low back pain and was seen by doctor. The employee was treated as first aid until 9-26-2011 when he began losing time at work

Cutting large rolled plate (20'X10'X1/4") using a cutting torch to cut plate to fit through entry of Floc Tank, plate was sitting up right while being cut, after cut was complete plate shifted & started to fall forward, employee grabbed plate to stabilize & keep plate from falling against him, lowered to ground level with help and strained/wrenched his back in the process.

We inadvertently didn't include this project as MSHA on quarterly 7000-2. As per MSHA directive, we have amended the 7000-2 for the 2nd quarter 2011 & are also filing an initial 7000-1 for this reportable injury. EE was rigging a beam. After it was raised, wind blew & the beam began swinging. EEs L hand was where the connection was to be made causing FX to L middle & ring fingers.

EE was using a bottle jack to help support and lift a detached rub rail of a miner. While jacking up, the rail, the jack moved. EE tried to hold the rail in place. The rail fell pinching EE left middle and ring finger against the ground resulting in a fracture of the tip of the L ring finger.
