Showing all 6Employee was climbing a small ladder to access the catwalk of the Deister return conveyor. Upon entering the catwalk the employee grabbed the handrails and transferred all of his weight to his left foot and heard a snap and felt a sharp pain in his left foot. The employee was taken to the local ER to be evaluated and was diagnosed with a fracture of the fifth metatarsal bone.
EE was shoveling under the primary tail pulley conveyor after the plant was shut down and he developed pain in his lower back. This was his normal job. The pain was bad enough that ee had it checked out at the emergency room and they said to rest for 5 days.
Employee was preparing product to be ground in a small test vessel size ball mill. While performing these duties, the cover to the ball mill fell from the table and struck the employee in the foot.
Employee got out of the haul truck (175), walked a short distance on the haul road. He started to bend down to pick up a rock & experienced lower back pain before he could touch the rock. The foreman took him to the emergency room at CUPH in Plattsburgh.
EE STRAINED HIS NECK WHILE SHOVELING. EE KEPT ON WORKING WITH NO RESTRICTIONS UNTIL 11/18/04. EE WENT TO DOCTOR AND WAS OFF THURSDAY AND FRIDAY (11/18 & 11/19). EE CAME BACK ON MONDAY (11/22) WITH NO RESTRICTIONS. I (SAFETY OFFICER) WAS ON VACATION WHEN EE WENT OUT. (EE IS A HUNTER.)
