While employee was disassembling metal shelving, he came in contact with a sharp corner. This resulted in a laceration to the right forearm that required four stitches.

Employee was bent over pre heating steel and when he went to standup he felt pain in his lower back (right side). He was diagnosed with strain/inflammation/prescription meds.

A contract employee, and a co-worker were in process of splicing a 6-inch water line with a metal band and banding tool. During the process, EE was pulling one end of the stuck band out of the banding tool with a pair of pliers. When the band sprang out, the sharp edge struck the side of his left hand, resulting in a laceration.

The injured was helping install a seat in a bulldozer. During the process, while his hand was underneath, the handle that latches the seat was inadvertently bumped which caused his ring and middle finger to get caught between the seat and base. As a result, his middle finger sustained a fracture at the finger nail area.

EE was in process of removing the transmission from truck at the truck bay in the main shop. While removing bolts from the cross member it suddenly fell catching a portion of his left hand between the cross member and frame. As a result, he sustained a laceration on the palm of his hand, at the base of the little finger.

EE reported on 2/9/07 that an alleged injury occurred on 2/8/07 while removing tire chain binders. He reported the alleged incident on 2/9/07 @ 6:10pm. He worked for 4 hours and sought medical attention @ 10:15pm. Managment questions this alleged incident/injury.

EEa contract employee for JP Technical Services, was in process of changing an `idler? on the 345 backhoe when his little finger caught a jagged piece of metal (on the side of the idler) resulting in a laceration which required seven (7) stitches.

EE was loading a train at the loadout and during the process traveled to the silo to turn the water on. While walking to the silo, he had to cross the track and over a rail car and while stepping up on the rail car step, his hard hat fell off. After bending over to pick up his hard hat, he rose up and struck his head on the bottom of the step, which caused a laceration.

Taking tilt hose loose, to allow nose cone to fall forward. Had to pry hose down out of the way. In doing so hose slipped hit Employee in the mouth, recieved 10 stitches to upper lip.

Injured and co-worker were working on a 785 rock truck engine. They had to work underneath the engine and were having problems trying to remove a piston. During the process, he overexerted or twisted his back while pushing up on a connecting rod. The pain grew worse as the shift proceeded. He worked the next shift but had to seek medical attention on Monday, 10/31/2005.

EE stated that on the morning of the injury, a tire had to be removed from his grease truck. As he was pulling on tire, he felt pain in his lower back area. No report was filled out at the time of injury; safety department was informed on July 11, 2005. Note: Employee did not start missing work until August 25, 2005.

Employee was working on a gasoline engine for a mechanics truck. As he was picking up the intake manifold, his little finger on left hand apparently struck a burr or sharp edge on the gasket. As a result, he sustained a laceration that required six (6) stitches to repair. He was able to return to work on the same day.

EE, injured, and co-workers had completed placing four (4) dragline bucket teeth on the shoe of the dragline. EE was in process of scooting them back when one tooth shifted, catching the top portion of his little finger on his right hand. He sustained a mashing type injury along with a broken bone.
