Employee approached and entered equipment doors at the slope bottom to exit the mine. Employee closed one door and turned to find the other door near them closing. Employee reached out to catch the door. Another employee was already inside the air-lock opened the out-by set of doors to exit the mine.

EE was unrolling felt liner and picked one end up to go over the dirt pile. EE felt a twinge in left shoulder. EE returned to work but did seek treatment on 01/07/2023. The x-ray showed no fractures or significant findings. Noted and EE returned without restrictions.

While removing top structure from overland belt the employee dropped the structure pinching right thumb between the structure and structure railing. This resulted in a fracture of the right hand.

While helping build a solid block wall bumped EE's left elbow. There was no visible wound, swelling or discoloration at the time. Reported swelling and inflammation 4/29/2021 and sought medical treatment for cellulitis. Began missing work after Diagnosis of and treatment for infection due to Septic Bursitis 4/30/2021.

While picking a piece of 1/4" steel plate measuring 17" X 25", and taking a couple steps, felt intense pain in EE's right upper arm. X-rays revealed that EE's right shoulder was dislocated. The shoulder was put back in place and EE's arm placed in a sling. EE began missing work Thursday, July 23, 2020.

While walking near a co-worker setting 6' props, was struck on top the right foot when a prop fell unexpectedly. The impact caused fractures to 2 Tarsal bones in EE's right foot. Treated and released to restricted duty.

While moving miner cable, felt a pinch in lower back. They went to pick up cable again and felt additional pain in back and into legs. They called for help to get out of the mine. They returned to work without restriction. They contacted a physician after allegedly aggravating the injury while sweeping with a broom and began missing work 10/25/2019.

While roof bolting, EE helped move a piece of shale that had struck EE's partner. The shale then flipped over while moving and struck EE on the right ankle. Initially reported as a first aid only 10/09/2017, EE began missing work after an MRI detected an osteochondral defect ankle sprain and corrective surgery was performed 01/29/2018.

While exiting the water truck and closing the door was holding the handrail with left hand when the mounting bracket for the rear view mirror attached to the door struck left thumb. Treatment included removal of the nail, suturing of the nail bed and re-attaching the nail. EE returned to unrestricted duty on next scheduled shift.

While recovering belt material and removing belt sail channels from the roof had the belt sail channel move unexpectedly. This movement pinched left index finger between a steel plate inside the channel and the Dewey Dag nut holding the sail up. The pinch resulted in a laceration that required sutures to close. EE returned to work the following shift to unrestricted duty.

While working and loading beams onto a belt caught ee left thumb between the beam and a standing prop. EE suffered a laceration at the base of ee left thumb that required sutures to close. EE was treated and returned to unrestricted duty finishing ee regular shift.

While working on the water pump stood up and leaned against a tool box. The box's lid fell closed and pinched his right thumb causing a laceration that required sutures to close. He was released to unrestricted duty and returned to work his next scheduled shift.

While loading 8 ft bolts onto a bolter felt pain in his left shoulder. He continued working, finished his shift and used vacation days not reporting the incident until 1/23/2015. X-rays were inconclusive and he was referred for an MRI. Became lost time when an MRI report was made available 2/4/2015 revealing a superior and posterior labral tear of the left shoulder.

Removing 1 of 2 covers above pump motor compartment, 1 hand on the side of cover, the other hand on 1 of the handles on top of cover. He pulled cover to him, lost his grip with his left hand, right hand was still in handle and twisted his arm. Released to go back to work next day. Primary doctor's opinion was he needed therapy and could return back to work 3/28/14 or sooner.

While helping to pull a loop of miner cable felt his stomach tighten. He continued working and didn't think anything of it, took his belt off later and did not feel anything. After he went home and was using the bathroom observed a bulge above his left groin. He completed this incident report on his next scheduled shift. After being seen by a specialist began missing work 9/9/2013.

While moving a piece of a sulfur ball from on top of a 4" waterline the rock, 24 X 16 X 4, moved much faster than expected pinching his left little fingertip against the roof. The pinch resulted in a laceration to the pad of the fingertip that required sutures to close. He was released to unrestricted duty.

While picking up bolts from the bumper of a truck, the tailgate fell open unexpectedly and struck the back of his right hand. He did not seek immediate attention and worked the remainder of that shift and the next two. Persistent pain and swelling prompted the completion of this report. X-rays subsequently revealed a fracture to the right index finger.

While setting 6' X 6' X 6' cribbing on top of steel sets was struck on the tip of his right thumb when the crib he tossed rolled back unexpectedly. The thumbnail was broken loose and X-rays revealed a fracture beneath the nail. He was treated and released to unrestricted duty reporting to work his next scheduled shift.

While roof bolting and installing an 8' bolt tapped the rotation and the 6' X 16' plate struck him on the right wrist leaving a laceration that required sutures to close. He was released to unrestricted duty and reported to work his next scheduled shift.

While hanging cable for a roof bolter the reel back lashed, pulling the cable out of hooks and striking the operator in the throat. Injuries involved lacerations to his lips and damage to cartilage in his throat. Treatment included sutures to his lower lip. He was released to unrestricted duty reporting to work his next shift.

After working in an awkward position carried several 5 gallon buckets of oil to fill the tank and felt pain in his lower back. The pain worsened overnight and he completed a report of incident and sought medical attention the following morning. Placed on a lifting, pushing, pulling and bending restriction until seen again by his physician 11/23/2011.

While attempting to change a tire on a bulk dust car and reaching behind the tire to pull it off was pinched through the fingernail of his right middle finger causing a fracture to the fingertip and leaving a laceration that required sutures to close.

While hooking a chain between a scoop and a disabled ram car had his right foot pinched when the ram car moved. He was transported by ambulance where X-rays revealed no fractures yet the removal of damaged tissue and the suturing of lacerations caused this to be a lost time injury.

The injured grabbed a rope attached to high line being dropped down a borehole. He was struck nearly simultaneously by a piece of wood that had been used to align the cable. The impact caused a puncture and laceration to the back of his left hand. X-rays revealed a fracture and he is awaiting consultation with an orthopedic surgeon.

While handling materials felt pain in his abdomen. Later felt tenderness in his right groin, he continued to work completing his shift. After work experienced discomfort prompting him to seek medical attention. Diagnosed with a right Lingual Hernia. He returned to duty with a 10 pound lifting restriction and desk work only with surgery expected to correct the condition.

While setting props and looking up got something in his left eye. At the end of his shift he attempted to flush the eye. EMTs assisted in rinsing the eye but irritation persisted prompting him to seek medical attention after he went home. He was diagnosed with a Corneal Abrasion and prescribed an antibiotic. He returned to work the following shift.

While shoveling felt a pain in his abdomen that descended into his groin area. Transported to St. John's ER and released to follow up with his own physician. Additional medical services were not available until Monday, 10/25/2010 when he was seen by MOHA and diagnosed with traumatic epydidimitis. He was placed on a "sitting work only" restriction 10/27/2010.

While loading a bundle of chain hangers on a mantrip they slipped and as he attempted to catch them pinched his right index finger against the ride blowing out the side of the finger. Transported to Springfield Memorial ER where X-rays revealed no fractures and sutures were used to close the wound. He was released to unrestricted duty and returned to the mine finishing his shift.

While recovering Kennedy stoppings pulled several panels down when material held by the panels fell striking the outside of his right ankle. Transported by private vehicle to MSHA Springfield where X-rays revealed a fracture to the right heel. He has not returned to work.

While installing an 8' bolt the corner of the plate rubbed against his right arm allowing a burr on the plate to slice his forearm leaving a laceration. He was transported to Midwest Occupational Health Associates where treatment included sutures to close the wound. He returned to work completing his regular shift.

While changing ram batteries and with batteries elevated reached beneath them to clean out coal and battery came down on his right hand causing crushing injuries. Transported by ambulance to Springfield Memorial ER where the index finger of his right hand was removed due to the extent of the injury. He was admitted for observation.

While cutting a section of rail on a cutting table and attempting to roll a six foot piece off the table it landed on top of his left foot causing pain and swelling. Transported by personal vehicle to Springfield Memorial ER where X-rays revealed several fractures in the foot. Treated and released to see an orthopedic surgeon for surgery to re-align the fractures.

While building a Kennedy stopping, the rail came out of the rib, striking him in the mouth and chipping a tooth. He finished working his shift; the chip was placed in a bottle of milk and then re-attached by his dentist. He has been released to unrestricted duty and did not miss any work.

While moving a battery cell it slipped pinching his right fingertip against a mantrip bumper causing a laceration. Transported by private vehicle to Memorial ER Springfield where X-rays revealed a fracture and sutures were required to close the wound. He was released to unrestricted duty and reported to his next scheduled shift.

Employee was adjusting the supply forks on the #38 scoop and had the sliding arm raised. He could not move it. After pulling several times and the arm not moving, he put it down. After he put it down his hand slid. When it slid his little finger on his right hand was sticking out. At this point, it was pinched between the fork prong and the support frame.

While moving a door felt pain in his lower back. The pain persisted and he saw a physician 2/5/2009 placing him off work. An initial report of incident was not filed until 2/8/2009 and not accepted until 2/11/2009. EE returned to work 2/11/2009 with minimal restrictions that can be accommodated by Viper's Flex Duty Program.

While assembling a drill feed chain on a roof bolter the pot moved pinching his right little finger causing a laceration and tearing the nail. X-rays revealed a fracture to the finger tip and the laceration was closed with sutures. Released with a `no underground work' restriction.

While lifting a hitch to pull a bolter he felt a pop in his right arm causing pain. Transported by private vehicle to St. John?s ER Springfield and diagnosed with a possible tear to the right bicep tendon. Returned to work on a no use of right arm restriction 9/30/2008 and is currently working flex duty awaiting further diagnosis and treatment.
