While installing pipe in the shaft, a small piece of metal entered a worker's right eye, causing irritation and redness. Employee was taken to an optometrist, the debris was removed and the employee was prescribed eye drops. At the time of the incident, the worker was wearing safety glasses.

Two employees were placing a section of steel brattice onto the Shaft 10 auxiliary conveyance. While positioning the brattice to allow the conveyance door to close, the injured employee lifted the material and tore their right bicep.

A crew was conducting auger drilling operations. The assistant operator went to retrieve a 4.25" auger from the storage rack and was struck in the left hand when auger shifted in the rack, pinching EE's hand between the moving and a stationary auger. Impact resistant gloves were worn at the time of the injury. The injury was a laceration to the outer left hand requiring 8 sutures.

EE was struck in the face with a long handle shovel after arguing with an employee over work duties resulting in a laceration.

The employee's finger was pinched between the sub-collar gate latching mechanism when a second employee assisted with opening the gate unexpectedly. The employee received stiches to close the wound.

The mechanic's left thumb was pinched between the VSM bucket and the cylinder while working to install the retaining pin. The injury required sutures to close the wound.

While at the kitchen sink, an employee squashed a bee against a window pane with a sponge. The window pane broke and the employee cut the back of EE's left hand. EMT was immediately notified and evaluated the employee. Employee was transported to the hospital and received three stitches and a tetanus shot.

The worker was rolling over a shaft guide. His finger was pinched between the guide and concrete floor. 4 sutures required to treat wound.

A drill crew was doing a wet trip. The helper was using a bucket to prevent the mud from spraying out of the pipe. The bucket was positioned on the pipe that was hanging in the elevators. As the helper pushed the rod back to direct the mud away, he cut his arm on the pipe that was still in the foot clamps.

Worker was removing a clamp from a vertical 8" pipe. Another vertical 8" pipe shifted, pinching the worker's small left finger between the pipes.

Worker opened a 4 inch valve on a charged airline to bleed it off. The force of the air releasing out of an 11 degree elbow caused the end of the pipe to move and it struck the worker on the left side of the nose. The edge of the safety glass lens caused a lacertation to the nose.

Water truck driver was task training another employee on the 8000 Gal Water Truck. He was trying to situate himself on the passenger's seat. He put his hand on the door jamb. When the trainee shut the door he closed it on the operator's thumb. He needed stitches on his thumb and also chipped the tip of his thumb bone.

Employee was logging core data into computer and needed to break core sample to view inside. Worker broke the core over an angle iron tray. When doing so, broken jagged end punctured his right hand between fore finger and thumb causing a laceration. Injury required 6 stitches to close.

Employee was lowering a sheet of tin off sampling building. Tin slipped from employee's grip and when he went to regain control of the sheet, the corner cut his left forearm.
