Employee was installing a sprocket. While doing so, the taper lock slid off of the shaft, striking the employee's finger, resulting in a laceration that required 4 sutures.
While removing a cylinder from the #2 kiln dust valve the employee caught the tip of right ring finger between the cylinder and the mounting bracket resulting in fracture.
Employee was removing a 4 inch ball valve to unplug a line. While doing so, the employee was splashed with caustic resulting in burns to the front of body.
With the assistance of another employee, an employee was carrying a countertop through a doorway. While doing so, the employee's hand was caught between the countertop and the door way resulting in a laceration that required sutures.
Employee in Precipitation stepped down into caustic material. Material entered the top of the employee's boots resulting in burns to both feet.
Employee was opening a valve at the top of Precipitation. While doing so, caustic splashed on the employee resulting in burns.
While taking the shifting cable off the barge, the cable pinched right hand. 1. employee failed to shift the barge into the correct position for cable release 2. employee placed hand in the pinch point between the cable and deck bit
Operator pulling on filter media sustained a back strain. Placed on restricted duty on May 9, 2016.
Employee was lifting an air tool stand with the assistance of another employee. While the employee felt slight back pain. The employee sought medical treatment 3 days later.
Employee failed to maintain proper footing when handling the rail switch
Employee was replacing a section of 6-inch pipe and caught right thumb between pipe and an object resulting in a tuft fracture.
The employee was rolling the blinds for the #3 PDS tank. While rolling the 16" blind the blind slipped out of the employees hand and smashed his finger between the blind and the pipe flange.
The employee was stretching out a hose when he felt a pain in left shoulder.
The associate from the Carpenter shop was removing scaffolding from behind 102 gantry. While removing the boards he had pinched his right pinky. The cut goes from the middle knuckle towards the hand.
The associate was in the process of tying up a ship, the spring lines were being moved with Gantry 101. The associates hand was in the chocker strap that holds the rope, the strap tightened up when it got stuck behind the dock bumper which casued the chocker to tighten up around the associates hand.
Associate was opening tank 71 with the beater valve, the chain wrapped around the tip of right ring finger. When he pulled on the chain he felt it squeeze his finger. Fracture sustained to right ring finger.
Associate was closing the bottom valve on WT64, when he felt a pop in right thumb. Causing a throbbing pain below his right thumb. Associate was taken to CORE (X-Rays were taken)and released with no restrictions. He was re-evaluated on 4/3/2014 by Orthopaedic Dr and a fracture was discovered. He was placed on restriction on 4/3/2014
Employee was switching low pressure heaters. While opening 4 west vapor valve, when the stem started to turn, employee took the locking pin and the stem of the valve. Employee's left thumb was lacerated the process. He received stitches.
Employee was pulling cloth over on 2B in the seed wash unit when he pulled something in his lower right groin area. He was diagnosed with a hernia and is unable to return to work at this time.
Employee was removing torque eliminator from a suction valve on AG7 when the valve slipped out of his hand and hit him on the left side of his upper rib cage. As a result of this injury he experienced severe pain and shortness of breath. X-rays revealed fractured ribs. He was given prescription medication and returned to work with no restrictions.
Employee was opening a valve to supply water to a 1-1/2" wash down hose. The nozzle had been left in the open position causing the hose to come off of the rack and strike him on the left middle finger. He has a tuft fracture to the distal tip on the finger.
Employee was getting an air hose and his right arm came into contact with the East 650 heater causing a 2nd degree thermal burn.
Employee was pulling on a chain to open a valve and felt a strain in his groin. He was evaluated and diagnosed with a hernia. His work duties were restricted on 6/11/13. He is scheduled for surgery to repair the hernia on July 2, 2013.
Employee was climbing a ladder to top floor to check the position of a Clarkson valve when he struck his left hand ring finger on a metal plate welded to a piece of pipe next to the ladder structure. His finger began to swell and cause pain. Finger is fractured. Employee's work duties restricted on 3-25-13.
Miner was tightening a union and felt a pain in his left forearm. Miner received first aid on date of injury. Referred to outside medical treatment on 2/21/13. On 2/22/13 employee had an MRI with negative findings. Miner recieved restrictions on 3/7/13. Diagnosis: distal bicep tendon rupture. Restrictions: do not use left arm or hand.
EE was pushing a roll around ladder into position to open the west cleaning caustic chain valve when he felt a pop in his right calf. Muscle strain.
Pulling on valves previous day. Woke up with abdominal pain. He was diagnosed with abdominal strain and was put on restricted duty.
Employee and a co-worker were replacing a 4" Clarkson valve. When he loosened the last bolt the valve shifted pinching his left ring finger causing a laceration requiring sutures. His work duties are now restricted.
Guarding was being installed when the choker broke striking employee on the shoulder and in the mouth. His lip was lacerated & required sutures. One tooth was completely knocked out of his mouth and a few of the other teeth were loose/damaged. His shoulder was bruised.
Cleaning 2 calcine pump screen box. As he attempted to open the bottom valve on wash tank 64 he felt a sharp pain in his lower right stomach/groin area. Restricted to No heavy lifting over 20#.
EE was replacing the cloth on 1B seed wash unit when he brushed burned his fingers on his right hand from the middle finger to the pinkie finger on the cloth. He covered them with bandages and returned to work. On 1/24/12 he reported to medical department with a 1cm area that was either avulsed or caustic had eaten away the tissue & swelling. He was given an antibiotic.
EE was replacing door on #1 belt. As he lifted the door he felt a pull in his left deltoid resulting in a shoulder strain. He continued to work without restrictions.On 11/15/11 Dr. diagnosed a possible partial SLAP tear. Dr. recommends surgery if not improved in 1-2 months. Surgery performed on 3/21/12. Lost Time begins today.
While placing an impact gun on a table with his left hand, employee states that he felt a "pop" in his left shoulder & then an intense pain. Suspected tear. He is not to use his left arm/shoulder. We cannot accommodate these restrictions, therefore this is now a lost time incident.
Employee was handed a scaffold board by a co-worker. A wood splinter entered his left hand at the base of his thumb through his glove. He had a medical procedure to remove the splinter on Tuesday, September 20, 2011. His work duties are restricted
EE was reinstalling a shaft. When he lifted the male end of the shaft, he did not notice that the universal was flipped up. When he rolled it, the universal folded down pinching his left pinky finger causing a fracture. His work duties were restricted. This incident was not reported to the Safety Department until October 23, 2011.
While cleaning the screen box for tank 65, EE felt something burning on his foot. He completed the task and reported to medical where he was diagnosed with a chemical burn and was given first aid treatment. He was later given a topical antibiotic prescription medication which makes this a medical treatment case.
On 4/8/11 EE was standing on a cover over the Lime Belt to remove scaffold. He lifted on the scaffold leg & felt a pain in the lower right abdomen.
EE moved three 55 gallon drums (partially filled with oil). When he picked up the drums, he felt a "pull" in his lower abdomen. Later that evening the pain increased. A hernia was discovered and he is scheduled for surgery.
EE was handling scaffold material overhead at #4 Alumina Load Out Track when he began to feel soreness in his right shoulder and between his shoulder blades. On February 23, 2011 he was given a Cortisone injection, making this case a Medical Treatment.
Employee was assisting GE factory representative to inspect bearings on #2 Steam Turbine Exciter work. He was pulling the bearing cap off to inspect the bearing. He lifted it up by hand and twisted around, pulling his back.
Employee was standing on a ladder, removing a 32lb. box filled with bottles of deodorizer from a 6' shelf. The box fell on his arm causing a torn tendon to his right bicep. He is scheduled for surgery on September 23, 2010 and will be out of work.
EE was moving a ladder stand when the eliminator fell and he tried to catch it. The eliminator hit his right middle finger & smashed it between the eliminator and the stand. His finger was x-ray'd at the local hospital and revealed a fracture and was given pain medication (non-prescription). He returned to work with no restrictions the same day.
EE was buffing off a weld and the buffer unexpectedly pushed his hand against the hot flange causing a thermal burn to the top of his left hand. He reported to medical, was treated and returned to work. EE was later provided with prescription medication. The burn, although small has caused some cellulitis in the tissue surrounding the burn which required medical treatment.
While moving ATV to a new boring location, wet mud began to build up on rear ATV tires. Employee got out with a shovel to try and remove the accumulated mud. The shovel became stuck in the mud, and when he attempted dislodge it, he states he felt and heard a "snap" in his lower back. He had a co-worker accompany him to the vehicle and drive him to the hospital.
Employee was opening #2 filter press which was taken offline by co-worker. He assumed that the press had finished draining but did not. When he opened it, hot liquor splashed right above his feet on the inside parts of his right & left calves causing 2nd degree burns.
EE was washing down and had to remove a piece of grating to allow himself to wash under it. He felt a slight pull in his lower back. He also began to feel numbness in his lower back. He was treated by medical dept. and returned to work with no restrictions. His symptoms worsened and he was referred to an orthopaedist. He was placed on work restrictions on April 12, 2010.
Employee was washing equipment & felt his left middle finger burning. He removed his glove and noticed a small burn, washed his finger, then reported to medical where he was treated & released to return to work with no restrictions. On 2/3/10, he returned to medical department with pain and swelling, was placed on oral antibiotics and returned to work with no restrictions.
Employee was reaching to open a ball valve & experienced sharp pain in his left shoulder before operating the valve. He reported to medical, was treated & returned to work with no restrictions. Employee continued to complain of pain. An MRI revealed tear in rotator cuff. He was referred to a specialist & was scheduled for rotator cuff surgery on 2/3/10.
EE was working in the slurry area and mud & liquor went over his boots. EE did not immediately remove his boots, but later went in the control room and showered, changed clothes and boots and returned to work. The next morning he reported to medical department. Both feet were burned on the top. He was taken to the burn unit in Baton Rouge, LA.
EE attempted to open a valve on tank 36. He felt an intense pain in the right shoulder. He was evaluated and underwent reduction of the right shoulder which was unsuccessful. He was then admitted to the hospital for observation and discharged on 10/27/09. First day of lost time will be 10/29/09. He continues off work at this time and will be reevaluated on 11/4/09.
