Employee was pulling on the bottom of tailgate. Trying to install a crib block between the gate and bed because the gate was sprung and would not latch. Employee pulled back on gate dislodging crib block, causing it to fall on top of EE's right foot. Causing fracture to top of right foot.

Showing all 35West section BK 53 509 scoop charger 2B belt Between #3 & 4 entry. Employee was setting up the fire ext. suppression tank back up on the charger and lost control of the tank dropping it on EE's left middle finger tip causing finger nail to be torn off. Went to ER and had a small tuft fracture and nail avulsion.
Employee was attempting to remove rock from boom/dump end of shuttle car. The rock slid pinching right pinky finger between the rock and the metal of the boom. Causing cut and break to top knuckle of pinky finger.
While cleaning out wet duster on scoop, the employee got water hose to spray it out and when they turned it on it slipped out of hand. The nozzle then flew back up striking the left upper lip. Cutting lip.
Employee was placing belt clamp on belt when clamp slipped and flipped up, striking EE in EE's mouth.
Employee was changing scoop batteries on scoop. As employee went to shut down the lids on the batteries, employee dropped the lid down on right hand.
Employee was closing large roll up door on new side of Plant when chain on door snagged on latch causing to wrap around left ring finger causing it to swell and bruise.
Employee was loading a timber on the ride, the timber started to roll off the back side of ride and EE reached out with right hand to try to catch it. This resulted in a fracture of EE's middle finger on the right hand.
Employee was helping load miner head onto low boy underground. Employee was in the process of centering the miner head up on the low boy. Employee reached under the miner head to get a chain and the crib block rolled over and struck employee's hand.
Employee was walking through curtain carrying tank of oxygen, as EE went through curtain EE smashed right pinky finger against rib.
Employee was hanging Shuttle Car cable to a cable hanger suspended to the roof and felt a pain in groin/back. Employee was transported to hospital. X-rays were negative but was placed off work until 5-23-2016.
The section crew was adding a 3rd bridge when the power cable drug under the 3rd bridge. The employee signaled the 3rd bridge operator to stop and when he did, the employee reached under the bridge and grabbed the cable. The 3rd bridge tilted and pinched the employees hand under the bridge.
Employee was taking drill steel out of the top. he dropped one of the drill steels & when he went to pick it up another steel fell out of the top & hit the employee on back of the head.
He was assisting one of Carter Roag's employees tighten a chain on a conveyor when the grease hose blew off putting grease in his face. He asked the employee of Carter Roag to throw him a rag as he could not see, he removed his safety glasses to use the rag and the rag that was gave to him was dirty and when he wiped his face and eyes it cut his eyeball.
Employee was helping carry Jack leg drill. His left leg gave out. Pre-existing condition pins popped in leg.
Employee was picking up a 15HP pump when he felt a muscle pull in his stomach. The employee saw his family doctor the following day and was diagnosed with a hernia.
Employee was opening an airlock door which had pressure on it, when the employee felt something pull in his right shoulder.
Employee was using his hands as a guide to pick up a rub rail and pin it on the continuous miner. The rub rail shifted and caught his hand between it and the miner, causing a laceration.
EMPLOYEE WAS OPENING A DOOR TO GAIN ACCESS TO THE BOTTOM DECK OF THE RAW COAL SCREEN. THE HINGES ON THE DOOR WERE RUSTED AND BROKE OFF, CAUSING THE DOOR TO DROP DOWN AND SMASH THE EMPLOYEES FINGERS.
Employee was putting a bottom deck screen in a scalping screen. Once the screen started to drop to the bottom deck, it flipped up on one corner and hit the employee across the bridge of the nose causing a laceration and a broken nose.
While removing section of heavy media pipe (approx. 10" dia. X 24" length) EE went to push on side pipe as it was being removed it came out swung around striking him on the right wrist/forearm, breaking wrist. (closed fracture)
Was hanging miner cable and hanger broke and cable fell on hand and broke left thumb.
EE setting air gap on hoist brake disc. the .003 gage shim slipped out of pack, the train shim slice the skin on the right palm.
While moving pipe from old scrappa at top floor loadout we had cut off. He was turning pipe and it had a sharp edge on pipe that cut through his glove and into his left index finger at app. the first bend of finger. Cut appeared to be about 1" across finger and could not tell how deep cut was.
Was pulling a rock off side of miner and rock smashed tip of finger between rock and frame of miner.
EE was working on getting a pin in tongue of a trailer and smashed his finger against the frame of batteries.
Was pulling slack on miner cable and felt pain in stomach. Had to have hernia repair surgery.
EE was putting first aid box in scoop on power move and caught rough edge with right thumb.
Removed fuel cap-sustained burns to face and hands.
While picking up a bottom belt roller on the #3 belt it slipped out of his hand and he mashed his right middle finger between the roller and a header board.
Injured had unloaded 4 foot resin bolts onto the ground behind the roof bolting machine. He threw a box of resin on the pile of bolts causing a bolt to flip up and hit him on the nose and left eye lid.
This individual was lifting and stacking 6" cement blocks when he felt a pain in his lower stomach area. Did not begin missing work until 1/19/06.
The injured was watching a mechanic pry a bearing off of an axle of 3 wheeler battery mantrip, when a piece of the bearing housing broke off striking his rt. hand
Back sprained while duck-walking carrying 40# of required equipment thru all entries of underground mine.
EE WAS ATTEMPTING TO LIFT MOTOR FROM MANTRIP TO PARTS TRAILER AND TWISTED HIS BACK.
