Showing all 3Employee was dislodging a rock from shaker/screen, and rock fell on top of left hand. Employee did not notify supervisor until Thursday, 1/26/17. Employee told supervisor hand was ok. 1/28/17 initial HR notification was made by supervisor with request for employee to be seen medically. Medical findings on 1/28/17 state no damage found or necessary medical treatment needed.
Employee was shoveling sand away from belt area of screen and injured lower back. Not sure how.
