Employee was assisting three others in replacing accuator arm using chain hoists. One man let go of his end and accuator fell on employee's shoulder and neck causing muscle strain. This was not reported to office until 9/12/05 when employee stopped work due to pain from this injury.

Employee was putting idler rollers on a belt & he was jerking on the belt roller to get it in place. Approx. 15 minutes later he states he felt pain in his back.
