EE was fabricating frame work in the fab area. A piece of steel fell over while EE was kneeling and grinding on the frame. The piece struck EE on the back of the head between the top of the welding helmet and the strap requiring stitches due to head laceration.

All PPE was being worn but a piece of slag dropped from the weld and rolled down EE's forehead and into EE's right eye coming to a rest on the bridge of EE's nose. Reportable due to prescription eye ointment applied. Released to work 4/22/2025
While replacing the head bearings on BL_CV05 EE thought they did something to their back. EE was in the process of lifting the bearing from their belt level to mid-chest when this happened.
EE was lining up an industrial pipe and used the back of EE's leg to help align it. EE felt a pain in left knee. Not reportable to MSHA until 2/21/24 when EE received a cortisone injection and medical treatment. Prior was able to work normal job duties and no medical treatment.
EE was unhooking a pin and releasing a lever on a semi-trailer. EE felt left shoulder tweak.
Shoveling a work area when their right shoulder started hurting.
WHILE PUSHING SCREENS UNDER SEMITRUCKS FOR SAND GRADING THE SCREENS KEPT FALLING OFF AND NEEDING TO BE PUSHED BACK. THIS REPETITION CAUSED EE'S WRISTS TO HURT. EE HAS A SPLINT FROM THE DR.
While changing screens sand fell past EE's safety glasses and got in eye. Only reportable due to a prescription ointment for EE's eye.
REPAIRING A SAND LINE CRUSHING BELT CAUSED SOME SAND TO FALL OFF AND GET INTO EE'S EYE CAUSING IRRITATION. EE DID NOT FEEL NEEDED MEDICAL ATTENTION UNTIL THE NEXT DAY WHEN IT STILL HURT. 9/8/22 EE SAW DR. AND GOT A PRESCRIPTION FOR EYE AND NO FOLLOW-UP WAS NEEDED. RELEASED TO FULL DUTY. FILED WITHIN 5 DAYS OF MEDICAL TREATMENT TO MSHA 9/12/22.
While hanging mesh from the JLG onto the mine ceiling EE overreached and stretched EE's shoulder causing injury. Removed from work on 6/28 to have therapy and a sling.
EE was changing out a pipe and was pushing it with EE's right arm to get it into the right location while reaching to move another pipe with EE's left arm. EE heard a pop and felt pain on EE's right side.
LIFTING SCREENS TO REPLACE FOR REPAIR IN THE MEGATEXTES AND FELT A TINGLING IN EE'S BACK. MEDICAL APT. ELECTED TO GO ON 1/21/22.
EE was pushing bolts and felt a pop in EE's back. EE did not have any pain immediately but EE's back started hurting overnight. EE reported 7/7/21 that EE was sore and wanted to see a Dr.
EE was assisting in loading holes for blasting and when done loading holes right shoulder was sore. This incident was not reported to a manager or HR until 3/4/21. After they went to personal Dr. and had been in therapy for 2 weeks. Due to therapy we are reporting.
EE was placing a pin on to a shelf when EE's hand slipped off the pin. The pin hit the shelf and proceeded to fall off and land on top of EE's right foot.
While EE was performing work of replacing a hose on a piece of equipment and also driving a pin back in EE got material in eye causing irritation. After multiple flushes it was still irritating EE and EE was taken to the doctor to have it examined- no foreign body was currently found or abrasion. EE received drops for eye and was released to full duty.
EE notified HR on 9/4/20 when HR called to verify EE was not continuing employment that EE received stitches on 5/12/20. EE was unable to provide the location of the incident or if there were any witnesses. EE was cutting and clearing trees and stated the saw was turned off but a limb kicked it back and struck EE in the leg giving EE 5 stitches.
EE was unloading a wire spook from the back of a truck and as the spool rotated it placed a metal sliver from the wire in wrist/palm area. EE requested medical attention on 4/3/20 and was released to full duty. EE received 3 stitches making it reportable.
EE was on restricted duty due to a personal injury to shoulder. EE felt a pop in shoulder climbing on the Titan. Investigation by Workers Comp. determined that the injury did not arise out of, or within the course and scope of employment 9/10. On 9/26 HR was notified by Workers Comp. that further investigation caused them to reverse the decision; injury occurred at work.
EE was attempting to clean out material built up in the tail pulley. While trying to free up the drop door dropped down knocking over the EE and landing on their leg causing injury.
EE possibly getting dust in eye causing an irritation. EE received eye drops making this reportable. EE stated EE worked in an area where dust was present then 3 weeks later notified HR that EE wanted to see the Dr. This was not reported immediately and is under review with Work Comp at this time. Date reported was 5/15/19 and dr was seen 5/16/19.
EE was shoveling at the Aggregate Crushing Line and felt lower back pain.
EE was cleaning out shotcreter when EE lifted the grate up and slammed it back down to knock off shotcrete. EE's finger got caught between the grate and grate support bracket as it came down.
EE and another co-worker pulled mesh to the kobota for them to take it to the heading in 22AX. When arrived at 22 they got out and unhooked the mesh, got back in the kobota to park it. EE then proceeded to walk to the JLG lift when groin started hurting on the right side.
EE picked up a cone and felt pain in EE's abdomen when sitting or bending over.
EE was pushing bolts. When lifting the bolt in the back to push it with the jackleg. As EE was lifting EE's shoulder started hurting. Now has a sharp pain in shoulder.
Employee was working on a belt splice replacement. As EE was pulling on the belt, EE felt a pain in the right shoulder.

EE was setting the brake on a rail car. When EE set the brake, EE felt a pain in the groin area.
EE was replacing a Grease Line and placed self in a poor posture position for a period of time. This resulted in back pain and therapy ordered.
EE was wearing safety glasses & when they removed the glasses they got something in their eye. Safety glasses had some sand on them as well as their gloves. Some sand got into the eye casing an abrasion on the eye. EE received a prescription for the eye.
EE was replacing a sheer pin on a semi-trailer 815. EE caught finger on the bolt which resulted on laceration. EE received stitches and a splint.
EE was moving a screen and when pushing a different screen one came back down and landed on another screen hitting EE's tail bone.
EE was pulling a screen to life into a telehandler and pulled back while performing this task. EE did not seek medical attention until 11-20-17. Notified HR 11-20-17 that ee wanted to see the DR. Filed 11-22-17 within 10 day time frame.
EE was pulling self out of a rail car and heard shoulder pop. EE did not seek medical attention until 10/24/2017. We are filing this 11/2/2017 within the 10 day reporting window.
EE was opening railcar lid, as EE raised the lid, the lid got stuck on the spout causing the lid to bounce and hit the top of EE's right hand.
EE used brake cleaner and Krol oil to clean a shaft. The product pooled in the key way and when EE inserted the key; the product shot up into EE's eyes. The chemical went under EE's safety glasses and prescription glasses. Reportable only due to prescription medication for eyes.
EE reported on 2-15-17 that shoulder was sore and that it is from doing the same movement every day. 2-14-17 was when it hurt the most. EE did not want to see the doctor and felt it was going to be ok. 4-10-17 EE reported to HR that EE was still having issues. An apt was scheduled for 4-11-17. EE received restrictions and return to work as of 4-11-17. Report filed 4-17-17
EE was trying to move an injured cat, the cat bit EE pinky finger on the right hand.
Apron on the HSI dropped, they shut down HSI, EE opened inspection door, hammer rolled and fell on EE's wrist and top of left hand.
EE was using the hose on the water truck and when he pulled on the hose the reel stuck and the hose didn't come out. This resulted in low back pain with restrictions.
EE was shutting the door of a piece of equipment and he had his thumb in the hinge. He shut his thumb in the door and resulted in breaking it.
EE was loading roof bolts onto the genie when one of them came loose off the hooks and hit his finger. Finger is broken on left hand
EE was picking up junk mesh and was smashing it up in small pieces and small piece punctured his right arm.
Bent over to pick up a can that fell on floor and felt a twinge in lower back. Any movement in back or walking makes lower back have a sharp pain.
EE locked out RTX, control room turned it off, tried pushing on it to get it to stop swinging, while doing this EE had his right hand on the swing arm and pinned his hand between bar and I-Beam. Notified employer on 10-22-14, he wanted medical care, restrictions given 10-22-14 at Dr.
EE was lifting a screen from under a semi with assistance from another person, but he felt his back pop and they set the screen back down
EE was opening the VSI lid and while closing it it wasn't moving freely. He had to work the lid open with bars after trying to pull it open. He didn't feel any pain until 6 am and then finished his shift with out any problems. EE didn't report injury and fill out report until 6-20-14. Doctors put on restrictions on 6-23-14 & Filed with MSHA 6-23-14.
EE was pushing a load with his right leg and felt something pop in his lower back.
