Employee was attempting to change a return roller on conveyor belt #4, when roller slipped out of his hand he reached to grab it causing a pain to left shoulder. On 5/30/13 after follow up doctor visit, doctor recommended surgery to shoulder.

Employee was placing wheel chock on chock hold on 980 loader, when his right hand slipped causing his left thumb to become caught between the chock and the chock holder. Resulted in fracture to employee's left thumb.

The employee was dumping sand down a waste sand chute. The sand back drafted onto the employees face. The employee was wearing all appropriate PPE. Sand debris got into the employees left eye. The employee was taken to a local doctor were he was given a preventive prescription ointment (TobraDex).

EE was leaving the dryer area and got sand in his gloves. The sand was hot and he dumped it out but ended up with burns on both hands. He was taken to the emergency room. No treatment was given but the doctor gave him a prescription to prevent infection and a pain killer. EE returned to work and has not missed any work.

EMPLOYEE WAS REMOVING A BOLT FROM A CUTTING EDGE ON A SKID STEER LOADER. THE BOLT HEAD WAS VERY SHARP AND CUT THROUGH HIS GLOVE. FOUR STITCHES WERE REQUIRED. HE RETURNED TO WORK.

Employee tried to move the Rotap and sound enclosure by himself. He stated he had made similar moves by himself several times. This time, he said he must have twisted the wrong way because he ended up straining an abdomen muscle and a groin muscle.
