Employee was checking tension on drive belts from caliche plant shaker. Employee locked out equipment and grabbed ahold of belts with right hand and when they pulled the sheave it shifted, catching employees hand between belt and sheave. Employee went to clinic where it was determined with X-rays of fractured index finger.

Employee with left hand grabbed the chassis and with right hand pushed the fork so it could return to original position, while doing so they didn't remove their hand and hit left thumb. Employee fractured finger.
