The employee was installing a hoist cylinder to the dump body of a 960E haul truck. He loosened the bolt on the shipping strap and the bolt threads failed causing the hoist cylinder to fall pinning his left shoulder between the hoist cylinder and the tire which resulted in shoulder trauma.

Shop clean up. Mechanic was moving items out of a wooden box and caught his hand/wrist on a metal shim as he was removing the item from the box which resulted in a lacaration requiring stitches.
