Employee was struck in the left leg by a screen that was dropped from an elevation. The screen contacted the handrail of the stairway, slid down the handrail, and struck the employee in the left leg as they were attempting to access the stairway. This resulted in an avulsion fracture to their left fibular head.

Showing all 3A SLC truck came onto the scales overweight so driver proceeded to dump off at the dump off area. Customer returned to the scales underweight and the driver said to the scale clerk to print the ticket. Driver exited the truck to retrieve the ticket and told the clerk had just cut thumb off. I've reached out to SLC for more information or details on this accident without success.
