Employee was using cutting torch to remove a damaged piece of 1/8" angle from the roof support on generator containment. Employee began removing by cutting the right side where it was connected to the structure. when the cut was complete, stored energy caused the angle to move hitting the employee in the lip causing a cut.

Injured was performing maintenance on a 14' screw conveyor and when employee turned the shaft manually his (R) pinky finger became pinched between the belt and the shive pulley. His fingernail became detached. Injured was taken to the Emergency Room and treated. The fingernail was reattached with two stitches to keep the nail in place while healing.

Employee was removing a pump from a 500 gallon above ground storage tank. As he was removing the pump diesel drained from the suction tube and splashed on the employees face which ran into his eyes.

Injured was assisting in lifting a portable stair with a total of five people. The lift was eight inches. When the group made the lift, injured felt pain in his lower back. He was taken to Baptist Medical Clinic in Little Rock, AR.
