Two employees were opening the hood of a dump truck. One of the employees stepped to the front of the truck to prevent the hood from moving too far forward and breaking the hinges of the hood. EE ended up taking the weight of the hood and this resulted in a small tear in EE's right bicep. EE is under lifting restrictions until an orthopedic appointment.

On 11/5/20 an employee was throwing sample bags into a dump hopper and felt EE's knee pop. EE's knee hurt for the remainder of EE's shift. At this point, EE feels EE does not want to seek medical care. The employee was taken on 5/26/2021 to Occ Med at Jackson TN. EE has been referred to an Orthopedic Physician in Paris for an MRI. EE received a prescription for Prednisone.

Employee was to load a bulk truck trailer at approximately 8:00 am. The employee stated that they opened the hatch with right hand. The left hand was resting on a rail or bar. The lid dropped onto a finger causing a laceration. The employee was taken to a local ER where they received 3 stitches.

An employee tried to dislodge a large cley knot, it spun and lightly pinned a hand against a beam. The leather glove bunched up and "pinched" the skin on the back of the right hand. When they pulled the hand away holding the knot the skin ripped away approx. 2 inches wide.

Employee was stacking bags from a bagging machine to a pallet. As the employee stacked a bag, EE felt a pop in EE's back. EE initially thought that EE had just turned the wrong way. After stacking two more bags the pain increased the employee reported to the supervisor and requested medical care. EE was placed on restricted duty.

Employee was pulling a rope that raises and lowers the Carbis safety cage on load out for bulk trailers. When pulling the rope to raise the cage from the trailer, ee right shoulder made a popping noise and immediate pain in shoulder. EE went to a physician on 2-9-2017 and was given restricted duty. EE returned to work on next shift.

Employee picked up sample from the back of a pick-up truck, extended left arm to sit the sample on the dock at which point EE's shoulder "popped". At this time pain is not such that employee wishes to seek medical attention. Over time employee stated EE is beginning to have issues with shoulder. EE is now on restricted duty as a result of Dr. visit on 6/8/16

EE left hand fingers were cut by a closing door while installing bags in the drier dust collector. EE received 3 sutures in 3 of his fingers.

EE cut his index finger on his left hand. EE was coming down the stairs from a silo when he caught his finger on a jagged edge on the metal handrail. EE was treated at East Wood Clinic Urgent Care in Paris and received 7 sutures.

EE was helping change a tire on a John Deere 1812C pull scraper, when the wheel hub slipped and cut his right index finger. EE was taken to a local clinic where he received 5 stitches.

EE was lifting a 50lbs bag of clay when he felt a pull in his left arm and started having pain. An Orthopedic doctor diagnosed EE with left elbow strain and treated with a cortisone shot. His arm was placed in a elbow strap and he was instructed not to use it until his follow up appointment.
