The employee and a contractor were removing an inspection cover from the top of the clinker cooler drag-chain. The employee's tip of pinky finger on left hand became pinched between the bottom of the cover and a metal flange as they placed the cover on the ground. The employee's fingernail was almost avulsed from the nail bed

Employee was pushing a small pile of material with their right foot in a sweeping motion, when their left foot slipped on loose material. Instead of falling forward, they leaned back and turned their body in a twisting motion, during which time they felt a pop in their knee.

As the employee was stepping out of the raw mill, the employee placed a hand on the raw mill tire hook and disrupted the accumulated dust causing it to become airborne. The airborne dust entered the airstream of the cooling vent fans and blew onto the employees face and under safety glasses causing dust to enter left eye.

Employee was walking backwards down the backside of the clinker dome access ramp when EE felt a strain in EE's right knee. The employee continued to experience pain in EE's knee and was referred to an orthopedic specialist for further diagnosis and evaluation.

Employee was opening the water truck cab door to open the water tank fill valve when the tip if EE's right fifth finger got wedged in between the cab door frame and the water fill tank valve resulting in a laceration.

The metal fork (1 inch X 24 inch) that supports the empty bags on the portland bag applicator was bent in a downward angle approximately 2 inches and as the employee attempted to lift up and straighten the fork the employee felt a pop below right shoulder

A production utilityman was cleaning the walkway along the lime/gypsum conveyor belt. While cleaning the walkway, they slipped on material and fell onto elbow causing a laceration requiring 4 stitches.

Employee was removing the bottom of an air slide pan that was with 3 other employees. They were using 2 come-a-longs to lower the airslide pan. The come-a-longs were then removed leaving the airslide pan supported by the pipe structure. The employees lifted the pan, the pan slipped and employee reached out at an awkward angle and felt discomfort in stomach.

At the request of the Alamo employee, a contractor tossed a 22 lbs. box of ceramic fiber insulation across the raw mill roller pair to the Alamo employee. The Alamo employee had left arm extended and lying across the roller pair seal air fan hose. The employee heard a pop in elbow area as arm hyperextended when the box made contact with the left arm.

EMPLOYEE WAS WORKING IN THE RAW MILL. EMPLOYEE WAS HOLDING A LINER PLATE IN PLACE AS THE BOLTS WERE BEING CUT OFF FROM PLATE FROM THE OUTSIDE. THE PLATE SLIPPED AND AS THEY ATTEMPTED TO GRASP THE PLATE IT SLIPPED AND CUT THROUGH THE GLOVE CAUSING A LACERATION ON THE LEFT HAND PALM THAT REQUIRED 4 STITCHES.

EMPLOYEE WAS LIFTING A GUARD COVER TO RE-INSTALL IT ON THE MASONRY PACKER CONVEYOR WHEN EE FELT A PAIN IN LOWER BACK. THE GUARD WAS IN THE UPRIGHT POSITION LEANING AGAINST THE BELT FRAME, BUT HAD A FOOT OF CEMENT AROUND IT CAUSING IT TO BE HEAVIER THAN IT NORMALLY IS.

EE was lifting a piece of metal plate flooring, approx 4'x5', weighing approx 100# to access work area. As EE slid the plate, EE lost grip & it dropped onto the fingers of EE's right hand. EE snatched EE's hand out from under the metal plate which caused the skin of EE's right forefinger to tear back, requiring 4 stitches. EE was not wearing any gloves at the time.

EE was inside the kiln moving a 2x12 board from across the kiln to the floor of the kiln. He picked up the board and tossed it to the side when it ricocheted off the rigging and landed on the top of his right foot causing his foot to swell. The ee was wearing steel toe boots at the time of the incident.

EE was walking down stairs carrying 2 empty 1 gallon buckets. He felt a pop in his right knee. He waited a few minutes and didn't hurt or bother him so continued on to collect samples. Didn't report until 2/9/15. Went to doctor & released to regular duty. Requested to return to doctor on 2/23/2015.
