The employee was attempting to clear a plugged pipe with a hose. The employee was working for an extended period of time and felt soreness in the right shoulder. The employee finished the shift and after a scheduled day off reported back to work and reported more pain in the shoulder.

Employee strained lower back while shoveling and cleaning under a hopper and conveyor belts for an extended period of time.
The employee did not lock tailgate after dumping raw sand and before being loaded with waste. After waste was added the tailgate opened. The employee lifted the tailgate to allow the material to fall and the tailgate to close. While reaching to lift the gate for a second time the material fell and the gate closed on the employee's left finger.
Employee was moving the wheelbarrow from underneath the screener. He grabbed the wheelbarrow with his right hand and was trying to use the other hand to move sand out of the screener to avoid more of a plug with a rake. He felt a needle like stab in his right shoulder.

Employee was working on installing a belt scale. He went up and down a ladder many times. Noticed some discomfort around 2 PM. More so when he got home an had rested awhile.
Employee was using a shovel to cleanup spillage around the wet processing plan. Later in the afternoon after repeated usage of the shovel he started to have some discomfort. *Note: Injury was not officially reported until after 4 PM on 4/19/13
