Employee was re-installing a bearing for a conveyor belt when EE felt something go in EE's eye. EE thought it was sand and didn't report it thinking it would flush itself out. On 7/21 employee's eye had become irritated and slightly swollen. EE was taken to MCH ER on 7/21 where a foreign object was removed. EE was given a full release.

Employee was manually moving the position of tripper 3 with another employee when they felt a strain around the Achilles of their right foot.

Maintenance was unloading an empty oxygen bottle from the maintenance truck to replace it with a full one. As they attempted to lift the bottle, they felt a sharp sensation in lower back and numbness in right leg and toes. Employee was taken to a local medical facility and evaluated. Employee was returned to full duty and upon a follow up appt, changed to modified duty.

Maintenance person tried to lift a sheave from the motor at sump 11. Once EE removed the sheave, EE turned to the right with the sheave in hand and felt a pinch in the back. Two co-workers did a beam lift to free EE of the sheave and notified the supervisor.

This was initially a first aid incident until the employee had surgery on 5/1/2018 and is now a restricted duty injury. Employee was rotating a sheave on a pump when EE heard a pop and felt pain in left calf. Originally a muscle strain.

Employee was tightening a ratchet strap, the strap slipped off and hit EE in the face causing a laceration to EE's chin which required several stitches.

Employee was gathering sand samples in a 5 gallon bucket. Bucket weight approx. 10 pounds. Employee removed the bucket from the bed of a pickup truck over the bed rail. Employee then noticed a pain in back that progressed through the day. Employee reported incident next day to supervisor and was taken to clinic. Placed on restriction until clearance.

Employee was using a bobcat to clear material around the dry plant. Employee was unable to close the door to the equipment and tried to force the door closed. Employee broke the glass which resulted in a laceration to right forearm.

A maintenance worker was rebuilding a 12X14 pump and case. While working on the pump, it slid off the pallet and landed on top of foot. The employee was taken to the emergency room by a co-worker. X-rays showed no fractures found in the foot. The doctor did not prescribe any medications at this time. The employee was taken off work after the follow-up appointment.

Employee was working under a conveyor belt, when ee stood up ee cut back on a piece of guarding, resulting in a couple of stitches.

Associate was washing hot sand from under the dryer. While doing so the splice on the hose he was using broke, causing water to hit the hot sand spraying sand and steam on the associate. The associate suffered first degree burns to his left arm, neck, and face.

The employee attempted to climb onto the tailings pond conveyor belt, when he felt a pain on his left knee. The employee was taken to local hospital for treatment. The X-rays were negative.
