The injured was in the process of removing a rail car from the 8 Right track onto the 3 East Main Line. EE was attempting to align the draw head on the car, when the rail gate slipped loose & fell catching the EE's rt hand between the gate & the draw head, causing a laceration to the palm of EE's right hand and bruise on top of EE's hand. The laceration required 10 stitches.

Showing all 6The injured was in the track entry between 25 rt. and 26 rt. when the incident occurred. He was lifting a partial barrel of chain onto a track mounted jeep when he felt a sharp pain in the bottom of his right foot.
The injured was carrying crib blocks to level storage unit for the longwall. He bent over and picked up a crib blocked and when he turned he felt something pop in his right knee. He sustained a sprain or a strain.
EE was lifting drill steel, when he felt a pain in his left shoulder. On 12/15/2014, EE underwent surgical procedure on left shoulder.
IE was working at the old bunker on 1 east. He was helping take roof jacks thru an air lock door. Once the they got the jacks thru the doors they were throwing them between two roof jacks that were already set to keep the walkway clear. The cuff of the IE glove hung on the jack and carried his hand into the jack that was set, causing a cut to the palm of his L hand, 6 sutures.
EE was helping change out 4" water line. Pipe got stuck in mud and ee picked up on end of pipe to free it. 1 of other EE's was on other end of pipe using a bar to help move his end over. Water line shifted and caught ee left ring finger between the pipe and a timber, causing a fracture. Now had left ring finger amputated at knuckle
