Miner was removing a slusher block from an eye pin at the face and felt something "give out" in left shoulder. Miner was diagnosed with a muscle strain of the left rotator cuff at the shoulder, and a sprain of the left AC joint. Cleared to return to work with restricted duty (no lifting above shoulder height with left arm). Recommended Physical Therapy evaluation and treatment.

Showing all 6While unloading 5 gal. water jugs from a flatbed truck EE felt pain in EE's right shoulder while lowering jugs. On 10/15/21 EE saw a physician, was diagnosed with right shoulder/elbow strain. Restricted duty (5 lbs. lifting max) and Physical Therapy Evaluation were prescribed. On 10/22/21 the physician increased wt. limit to 15 lbs., and recommended Physical Therapy Treatment.
While removing a compressed gas cylinder from a storage rack, the cylinder slipped and smashed an employee's right pinky finger between the cylinder cap and cylinder rack. The event resulted in three stitches to the finger nail bed.
Employee and partner were installing pipe for utilities when the partner slipped and dropped the pipe which caused the employee to drop their end of the pipe, dropping it on their foot.
The Miner tried to avoid water coming out of the back. His ROPS struck the rib kicking a rock loose that struck his right forearm.
Employee was shoveling out of ball mill feed and heard felt a strain in his shoulder. Employee was evaluated at medical center and returned to duty next shift with restrictions. No lost time.
