The employee was helping another employee move a Winchester Ringbuster kiln gun from the kiln deck to the storage area of the mine. The gun discharged while the employees were lifting it. This resulted in the slug striking the employee in left foot.

While pulling the 8 gauge shotgun stand out of the #2 hood after shooting a ring, one of the wheels of the shotgun stand came apart and made it more difficult for the employee to pull the gun. The employee continued pulling the shotgun stand and twisted left knee.

Employee was pulling a water hose in a man-lift. EE was attempting to wash the number #2 scrubber tank. As EE tried to lift the water hose, EE felt a pain in lower back. Employee went to the doctor on 1-23-17. The doctor released EE to regular duty without restrictions on 1-23-17. EE went to EE's own doctor on 2/3/2017 and was put on restricted duty.

Employee and a co-worker were removing port-a-power heads from under kiln. The third and last one slipped out of co-workers hand and employee could not get their hand away in time and the head caught the end of EE's finger. The tip of the middle finger was broken and lacerated by the 275 pound port-a-power head.

Employee strained his shoulder while pushing a guard back in place. Employee did not report injury until the following Monday. After his first visit to the doctor we was NOT given restrictions. He went to is own doctor and was put on restrictions weeks later.
