Employee was helping remove a spindle group from a front suspension cylinder of a haul truck. When the spindle came loose it dropped down striking the employee's arm and caught it between the spindle and the steering link of the haul truck resulting in a deep laceration to the left forearm.

Employee was lifting a fire extinguisher up on a dozer with their right arm they swung it up onto the track then up on the hood. They completed the shift, went home and stated that they then noticed their arm didn't look right but had no pain. They reported it the next day but continued to work until 12/18/2025. They were then was taken off work to repair an upper arm injury.

Employee stated that EE and another employee were installing belt hangers in the belt entry on the section. When EE was pushing the bolt up into the top, the 2-foot drill wrench EE was using broke and a piece of it struck EE in the mouth causing a laceration to EE's upper lip. The employee received 3 sutures in EE's lip and returned to work on EE's next scheduled shift.

Employee stated that EE was breaking the metal bands on a pallet of 12' timbers when one of the timbers rolled off the pallet and struck EE's right foot. Employee saw a doctor and was not diagnosed with an injury till a later date.

Injured Worker was in process of dismantling water line from roof of mine when the load of the water line on Injured Worker's shoulder was too heavy, causing Injured Worker to drop water line to ground and began feeling pain in stomach area.

Employee stated was attempting to hang line curtain when EE felt a pain in the left shoulder. Injured employee did not file a report until the following Monday (11/13/2023). After filing the report, EE drove self to Raleigh General where EE was taken off work for 5 days. Employee remains off work, but due to medical diagnosis we now feel that EE meets the Part 50 criteria.

Employee stated that the employee was finishing a stopping that the day shift had started to build. Employee was up on a ladder when the stopping fell and struck the ladder, knocking employee down to the mine floor, where a piece of block struck employee on the left foot. Employee was diagnosed with a fractured bone in left foot.

Employee stated that EE and another employee were attempting to install a slide in a rock box when the metal they were moving slipped out of EE's hand and struck EE's knee against the side of the rock box.

Employee stated they were mounting the 830 rock and when their weight was on the steps they pushed back under the catwalk, and they had to use their arm strength to pull themselves up causing a sharp pain in their right shoulder.

Employee was lifting belt structure out of an insert when they felt pain in right knee. Employee was diagnosed with a sprain and placed off work. During preparation for a Pt. 50 Audit it was discovered that this incident was not reported to MSHA but was reported internally as an NFDL. This incident was counted as an NFDL on the 7000-2, submitted for the mine for Q4 2022.

Employee stated EE was at the section tailpiece attempting to remove the water manifold from the waterline using a crib block to strike the manifold when EE's hand slipped mid swing, and EE mashed EE's right middle finger between the crib block and the cut off valve for the bull line.

Employee stated that they were lifting a 5 gallon can of hydraulic oil out of a scoop bucket and felt a pain in stomach. The employee finished the shift but did not return to work. They called the mines the next day and informed management that they were referred to a surgeon. The employee returned to work on 7/18/2022.

Employee was attempting to unthaw a hose from a lube truck. Employee had poured some anti-freeze & conditioner into a reservoir that would be injected into the stopped up hose. The employee did not replace the cap back on the reservoir resulting in the fluid coming in contact with both eyes. The employee was transported to an area hospital and treated.

Employee sought medical treatment after experiencing back pain while unloading trash. Operator notified on 8/16/21 that employee was diagnosed with pre-existing degenerative spondyloarthropathy, a form of arthritis, after results of an MRI indicate no significate changes from a previous MRI conducted on 6/2/2020.

Witness stated that themselves and another were in the process of moving a speed reducer across the 4th floor of the prep plant. Witness was using a slate bar to walk the speed reducer and injured employee was pulling the speed reducer with a strap. Injured stated that on the 5th pull they felt a sharp pain in lower back.

Employee stated that EE was attempting to get a can of hydraulic oil from the oil sled. The cans were stacked 3 high, when EE pulled the top can off a second can fell from the 2nd row landing on the side of EE's right foot resulting in a fractured little toe.

Employee stated that EE was in the process of removing coal that had built up on the side of EE's shuttle car to help EE's visibility. EE stated that when EE pushed the coal into the shuttle car EE's right forearm was cut on a sharp edge of the sideboard caused by wear and tear.

Employee stated EE was pulling C/M cable in the #7 entry when EE felt a sharp burning pain in EE's lower back. Employee continued to work the remainder of the shift. On EE's way outside, the employee stated the pain got worse. EE went to the bathhouse after getting outside, and then returned to the mine office to report the incident before leaving mine property.

Injured employee stated EE was operating the left Fletcher bolter on the #1 section, stated that EE was installing 10ft cable bolts in the intersection between entries 3 & 4 at #251 break, stated EE was 6ft in depth for a 10ft deep hole, stated that when bolt partner dropped pot it caused stress on EE's steel causing it to break and therefore strike EE's left pinky finger.
