Employee was helping remove a spindle group from a front suspension cylinder of a haul truck. When the spindle came loose it dropped down striking the employee's arm and caught it between the spindle and the steering link of the haul truck resulting in a deep laceration to the left forearm.

Employee was lifting a fire extinguisher up on a dozer with their right arm they swung it up onto the track then up on the hood. They completed the shift, went home and stated that they then noticed their arm didn't look right but had no pain. They reported it the next day but continued to work until 12/18/2025. They were then was taken off work to repair an upper arm injury.

Mechanic was attempting to install a pin into the retaining cable for a Cat 785 rock truck when the cable slipped causing an incision in mechanic's right hand

Employee stated that EE and another employee were installing belt hangers in the belt entry on the section. When EE was pushing the bolt up into the top, the 2-foot drill wrench EE was using broke and a piece of it struck EE in the mouth causing a laceration to EE's upper lip. The employee received 3 sutures in EE's lip and returned to work on EE's next scheduled shift.

Employee stated that employee was attempting to raise the shield on the shuttle car when employee felt a pain in right shoulder.

Employee stated that EE was breaking the metal bands on a pallet of 12' timbers when one of the timbers rolled off the pallet and struck EE's right foot. Employee saw a doctor and was not diagnosed with an injury till a later date.

Injured Worker was in process of dismantling water line from roof of mine when the load of the water line on Injured Worker's shoulder was too heavy, causing Injured Worker to drop water line to ground and began feeling pain in stomach area.

Employee stated was attempting to hang line curtain when EE felt a pain in the left shoulder. Injured employee did not file a report until the following Monday (11/13/2023). After filing the report, EE drove self to Raleigh General where EE was taken off work for 5 days. Employee remains off work, but due to medical diagnosis we now feel that EE meets the Part 50 criteria.

Employee stated that the employee was finishing a stopping that the day shift had started to build. Employee was up on a ladder when the stopping fell and struck the ladder, knocking employee down to the mine floor, where a piece of block struck employee on the left foot. Employee was diagnosed with a fractured bone in left foot.

Employee stated that EE and another employee were attempting to install a slide in a rock box when the metal they were moving slipped out of EE's hand and struck EE's knee against the side of the rock box.

Employee stated they were mounting the 830 rock and when their weight was on the steps they pushed back under the catwalk, and they had to use their arm strength to pull themselves up causing a sharp pain in their right shoulder.

Employee was lifting belt structure out of an insert when they felt pain in right knee. Employee was diagnosed with a sprain and placed off work. During preparation for a Pt. 50 Audit it was discovered that this incident was not reported to MSHA but was reported internally as an NFDL. This incident was counted as an NFDL on the 7000-2, submitted for the mine for Q4 2022.

Employee stated EE was at the section tailpiece attempting to remove the water manifold from the waterline using a crib block to strike the manifold when EE's hand slipped mid swing, and EE mashed EE's right middle finger between the crib block and the cut off valve for the bull line.

Employee stated that they were lifting a 5 gallon can of hydraulic oil out of a scoop bucket and felt a pain in stomach. The employee finished the shift but did not return to work. They called the mines the next day and informed management that they were referred to a surgeon. The employee returned to work on 7/18/2022.

Employee was attempting to unthaw a hose from a lube truck. Employee had poured some anti-freeze & conditioner into a reservoir that would be injected into the stopped up hose. The employee did not replace the cap back on the reservoir resulting in the fluid coming in contact with both eyes. The employee was transported to an area hospital and treated.

Employee was stemming blasting holes using a shovel and twisted EE's back.

Employee sought medical treatment after experiencing back pain while unloading trash. Operator notified on 8/16/21 that employee was diagnosed with pre-existing degenerative spondyloarthropathy, a form of arthritis, after results of an MRI indicate no significate changes from a previous MRI conducted on 6/2/2020.

Employee was mounting a grader when employee placed left foot into the bottom step/foothold on the ladder and pushed off with right leg that was still on the ground. Employee felt discomfort in right knee after pushing off.

Witness stated that themselves and another were in the process of moving a speed reducer across the 4th floor of the prep plant. Witness was using a slate bar to walk the speed reducer and injured employee was pulling the speed reducer with a strap. Injured stated that on the 5th pull they felt a sharp pain in lower back.

Employee stated that they were helping an electrician fix a ride and they and another employee lifted the ride up. When they let it down employee got left thumb caught between the ride and a bottle jack resulting in 3 stitches.

Employee stated while attempting to remove the cat idler on the operator side it got stuck. When it freed up it fell on employee's right pinky finger smashing it causing employee to receive 2 stitches.

Employee stated that they went outside to get a case of drinking water to put in the meeting room. They lifted the case of water to set it on the table when they herd and felt a popping in the right shoulder.

Employee was working on a D10 dozer idler when employee was trying to get out employee had lifted self up and sat on the push arm and twisted body to throw feet up on the push arm when employee felt pain in lower back.

Employee stated that EE was attempting to get a can of hydraulic oil from the oil sled. The cans were stacked 3 high, when EE pulled the top can off a second can fell from the 2nd row landing on the side of EE's right foot resulting in a fractured little toe.

Employee stated that EE was in the process of removing coal that had built up on the side of EE's shuttle car to help EE's visibility. EE stated that when EE pushed the coal into the shuttle car EE's right forearm was cut on a sharp edge of the sideboard caused by wear and tear.

EE stated that EE was loading a belt cradle into a mantrip to take to 52 Bk. As EE slid the cradle into the mantrip, EE felt a sharp pain in middle/lower back.

Employee stated EE was pulling C/M cable in the #7 entry when EE felt a sharp burning pain in EE's lower back. Employee continued to work the remainder of the shift. On EE's way outside, the employee stated the pain got worse. EE went to the bathhouse after getting outside, and then returned to the mine office to report the incident before leaving mine property.

Employee was lowering down a counter weight when they slipped on spillage. Employee worked up until 10/31/2019...went off on comp on 11/01/2019.

Employee stated EE shut c belt off to remove a rock that was fouled in the structure as EE lifted the rock onto the belt EE felt a sharp pain in EE's groining area

Employee stated they were carrying a reel and fender over to shuttle car in front of buggy and felt a sharp pain in their groin. Employee sought medical treatment and was taken off work.

Injured employee stated EE was operating the left Fletcher bolter on the #1 section, stated that EE was installing 10ft cable bolts in the intersection between entries 3 & 4 at #251 break, stated EE was 6ft in depth for a 10ft deep hole, stated that when bolt partner dropped pot it caused stress on EE's steel causing it to break and therefore strike EE's left pinky finger.

Employee stated was cleaning out the deck of the shuttle car when EE re-injured an old injury in EE's middle back area. Employee called in the following day.

Employee stated employee bent over to pick up a bundle of plates when employee felt a pain in employee's back. Employee sought medical treatment and was taken off work.

Employee was loading a hydraulic cylinder into a service scoop bucket and felt a sharp pain in the RT Shoulder. The employee finished the shift but the pain worsened through the night and EE drove self to the hospital after working.

Employee stated EE was moving a 72" belt structure out of a roadway, employee bent over to move structure and felt a pain in EE's lower stomach area.

Employee stated was hanging miner cable with the assistance of 4 others when EE re-aggravated a lower back injury. Employee did not work the following shift.

Employee stated was removing a pin out of a cutter chain. As it was beat out the employee caught the pin and mashed right index finger between pin and shear arm. Employee received 3 stitches.

Employee stated EE was in the process of plastering a stopping when a glob of plaster splattered off the stopping onto EE's glasses into EE's eyes. Employee sought medical treatment and was given medicated eye drops due to irritation.

Employee stated EE was helping move CM cable out of the way when MRS cable pulled down and struck employee in the back of the head straining EE's neck. Employee sought medical treatment on 10/30/2018 and was given three days away from work.

Employee stated ee and another employee were attempting to place a strap on the T-bar when ee was cut by the strap on wrist during the exchange. The employee received 10 stitches and was placed off work under doctor's orders.

Employee stated ee was carrying a timber from the scoop bucket to the rib when legs felt weak and ee fell to the ground. Employee complained of back spasms.

Employee did not make notification of incident until 10/24/18. EE sought treatment on 10/26/18. Employee was attempting pick up crib block that was stuck in mud when EE felt a strain in the groin area.

Employee stated ee was helping hang miner cable when ee felt a sharp pain in right shoulder. Employee sought medical treatment and was taken off work under doctors orders.

Employee stated was lifting a tool bag from ground level to the top of a scoop when employee felt a sharp pain in back. Employee was taken off work under doctors orders.

Employee was loading tray on bolter when EE struck EE's right elbow against the bolter resulting lost work days.

Employee's right ring finger was caught between the lid of an SCSR Cache resulting in a laceration that required 11 sutures.

Employee stated EE was shoveling at a belt head when EE felt a pain in EE's lower back resulting in a sprain/strain injury and lost work days.

Employee stated a bearing housing fell on left foot resulting in a fracture and restricted work days. Employee did not seek medical treatment until 3/11/2018 at which time incident became reportable.

Employee picked up a wooden pallet and twisted with feet planted and felt a pop in the left knee which resulted in a sprain/strain injury and lost work days.

Employee stated EE was lifting a belt scraper when EE felt pain in EE's lower back and both legs resulting in sprain / strain injury and lost work days.
